Responsible To: Assistant Director of Care - Direct Care

Job Description:

Are you a compassionate and dedicated LPN with a passion for delivering quality medical care? We're looking for a Physician Liaison to join our team in Berwick, Nova Scotia. In this role, you will collaborate closely with the Medical Director and interdisciplinary teams to ensure the best medical care for our residents, from assessments to medication reviews, specialist coordination, and diagnostics.

As a Physician Liaison, you'll play a critical role in managing the flow of care between our team, residents, and families, ensuring all necessary medical documentation is accurate and up-to-date. You'll also oversee backup medication administration records (MAR) and ensure continuity of care during disruptions.

Key Responsibilities:

  • Collaborate with the Medical Director to ensure comprehensive care planning, treatment, and follow-up.
  • Prepare for physician rounds, gathering and documenting resident health history.
  • Oversee medication orders, referrals to specialists, and lab result tracking.
  • Facilitate medication reviews with physicians and pharmacists to optimize treatment plans.
  • Manage the Iron Key system for backup MAR during internet/power outages.
  • Co-chair the Pharmacy Committee, focusing on medication management best practices.

Qualifications:

  • LPN license in good standing with the College of Nurses of Nova Scotia.
  • Strong communication skills and attention to detail.
  • Experience in long-term care settings preferred.
  • Ability to maintain confidentiality and privacy at all times.
  • Strong organizational skills and the ability to collaborate with various healthcare professionals.
