Tideview Terrace, an Eden Alternative Registered Long Term Care Facility located in Digby NS, serving 90 residents and operates on an annual budget of $12M. We are seeking a licensed regulated health care professional (RN, OT, PT, Dietitian, SW) to act as the Resident and Family Engagement Coordinator, in a full time position and part of the management team reporting to the Professional Practice Manager.

The Resident and Family Engagement Coordinator is responsible for facilitating a smooth transition for residents and their families into Tideview Terrace, ensuring that all necessary supports, resources, and documents are in place. This role requires collaboration with the nursing team and other healthcare providers to ensure the ongoing wellbeing and engagement of both residents and families. The coordinator serves as an advocate, guiding residents and their families through all stages of care, from move-in to end-of-life support, while facilitating key services including the coordination of appointments and respite visits.

Additionally, the coordinator leads family gatherings, educates families on policies and procedures, and connects them to external resources. They play a pivotal role in ensuring that the voices and needs of residents and families are represented and reflected in the care provided. This role requires a compassionate individual who can work collaboratively with multiple interested parties while maintaining transparency and open communication. All tasks are to be approached in a spirit of teamwork, within and among departments, to ensure cooperation and completion in an efficient manner. The Resident and Family Engagement Coordinator must possess strong organizational, interpersonal, and advocacy skills, ensuring that the Mission, Vision, and Values of Tideview Terrace are upheld in all aspects of resident and family engagement.

RESPONSIBILITES:

  • Facilitate all resident move-ins by ensuring supports, equipment, financial information, documentation, education, and all other appropriate resources are in place
  • Collaborate with nursing and recreation teams to facilitate resident move-ins and provide a supportive transition to long term care for residents and families
  • Liaise with residents, family members, other health care providers, managers, and consultants, acting as a partner in the care of residents and families, sharing relevant information and sourcing best practices to serve and address the expectations of residents and families
  • Orient residents and families to the home and ensure they are familiar with the environment and the staff as well as existing policies, procedures, and standards
  • Provide on-going support to families throughout the duration of their relationship with the home
  • Collaborate with neighborhood managers and/or primary nursing staff for complex care/family situations
  • Act as an advocate to residents and families ensuring their voice is represented and reflected in all the work at the home
  • Lead the regularly scheduled family and friends gatherings providing education and supports to families
  • Connect families with external community resources and supports
  • Assist residents and families with the establishment of SDMs, POAs and advance directives
  • Provide end of life support to families and facilitate move out planning
  • Participate in initial and annual resident care conferences
  • Coordinate and schedule all respite visits to the home
  • Coordinate all resident appointments ensuring all transportation, materials and human resources are in place.
  • Facilitate all requests for Medical Assistance in Dying (MAID) and support residents and families through this journey.
  • Take a lead role in supporting and investigating any allegations of resident abuse under PPCA
  • Utilize continuous quality improvement principles and evidence-based decision-making processes to ensure delivery of quality care and services to residents and families
  • Complete and support data analysis within Tideview Terrace's digital platforms as it relates to this work
  • Assist in establishing standards for measuring the quality of resident and family experience through a Quality/Risk Management Program. Assist in implementing related department goals and objectives (short term and long term), in keeping with the Mission, Vision and Values of Tideview Terrace as an Eden Registered facility
  • Supervise and participate in the performance management of the Nursing team in care delivery, delegating and reallocating staff as needed
  • Continuously communicate with staff, supporting staff, and assisting in troubleshooting/problem solving as needed. Report any issues/concerns to the Manager and follow up as directed
  • Represent Tideview Terrace with residents, families and the public. Maintain transparency in communicating with families, ensuring open, honest, and timely communication is shared

Qualifications:

The ideal candidate will have the following qualifications:

  • Bachelor's degree in Nursing, Social Work, Nutrition, Physiotherapy or Occupational Therapy
  • Current and ongoing registration with the Nova Scotia Regulated Health Profession Act
  • Minimum two years of professional experience and previous long term care experience is considered an asset
  • Previous supervisory and teaching experience is considered an asset
  • Demonstrated leadership skills
  • Various Training/ Certification in elder care, leadership / management
  • Prior training as a Certified Eden Associate, Vivage Neighborhood Guide, Person Centered Leadership Model, Art of Hosting, LEAN in LTC, PATH, and InterRAI considered an asset.
  • Solid communication skills and proven ability to work collaboratively with others
  • Service orientated with ability to build relationships and create win/win situations
  • Excellent listening, negotiation, team building and problem-solving skills
  • Ability to negotiate sensitive conflict resolution situations
  • A "big picture" thinker and change agent with a propensity for continuous learning
  • Proven ability to manage multiple projects and meet deadlines
  • Proven ability to make timely, effective decisions in a fast paced, changing environment
  • Solid computer literacy skills including proficiency in MS Suite of applications such as Word, Outlook and Excel, Microsoft 365 and MSTeams, as well as clinical applications including DIS, InterRAI using Momemtum, eMAR
  • Flexibility to work non-standard hours if required
