Job Summary
The primary purpose of this position is to provide professional and clinical leadership, supervision, consultation monitoring and support in the planning, development, coordination and evaluation of Social Work practice within the Golden Heights Manor/PCU Bonavista. The Social Worker II is a member of the Interdisciplinary Care Team and is responsible for the delivery of Social Work services to the Residents/Families at Golden Heights Manor/PCU Bonavista. The Social Worker II is responsible for the coordination of resident admissions They will be required to provide field supervision to Social Work students. This position will work in collaboration with all members of the Interdisciplinary Care Team to achieve operational and resident-centered care objectives. Will be expected to provide relief coverage for acute care at Bonavista Peninsula Health Center.
This position reports to the Regional Manager - Allied Health and maintains accountability to the Social Work Professional Practice Consultant for professional integrity and professional practice issues.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador Association of Social Workers (NLCSW) is required.
Minimum of three years of experience within the last five working as a Social Worker in an acute care program is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act
Knowledge of discharge planning and community-based resources.
Knowledge of palliative and end of life issues.
Knowledge of crisis interventions and supportive counseling.
Knowledge of case management.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated ability to work with a high degree of independence and initiative.
Demonstrated strong program development and evaluation skills.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated ability to work effectively within an interprofessional environment, and establish and maintain effective working relationships.
Demonstrated strong awareness of ethical principles and standards of professional conduct and practice.
A Condition of employment includes a valid Class 5 Newfoundland Driver's License and the use of a personal vehicle.
