RESIDENT CARE COORDINATOR (SW, OT, RN, or other suitable and experienced Regulated Health Professional)
PERMANENT FULL TIME
Queens Manor, a 61-bed long-term care facility located in Liverpool, Nova Scotia, is seeking an innovative and enthusiastic Regulated Health Professional to join its leadership team as Resident Care Coordinator. This is a newly funded position. As an integral part of the multi-disciplinary team, reporting to the Director of Resident Care, you will provide a combination of services in the areas of admissions, resident and family support, and care coordination. Your role will contribute to Queens Manor's vision of a long term care facility that "adds life to years."
Responsibilities
The Resident Care Coordinator assumes the responsibility to:
· Coordinate admissions to the facility, answer inquiries from family members and general public on admission procedures
· Assists with the admission, transfer and discharge of residents in coordination with the interdisciplinary team
· Provide therapeutic crisis intervention as required
· Provide counseling and therapy to residents and families as appropriate; develop support groups as necessary
· Facilitate resident care conferences, team meetings and family meetings; participate as a member of the interdisciplinary team
· Prepares funding requests and reports relating to resident needs; maintain resident records as required
· Develop and maintain relationships with Department of Seniors and Long Term Care, Nova Scotia Health, community institutions and community groups as necessary
· Provide in-services and education sessions to residents/families and staff on a variety of topics
· Ensure resident safety by following and applying all standard operational procedures, safe work practices and training
Qualifications
· A Regulated Health Professional in good standing with their regulatory body.
· A skill set within the scope of your profession that is congruent with the responsibilities listed above.
· Excellent verbal and written communicator
· Proven team player open to collaboration and demonstrating a "growth mindset"
· A willingness to create solutions when faced with challenges
· Five years experience in long term care or acute care environment preferred but not necessary to apply and be considered
· Demonstrated genuine interest and passion for the aging population within a "resident-centered" care philosophy
