Definition of work:
This is an advanced professional level position involving a high degree of responsibility, autonomy and independence in identifying and providing specialized assessment, individual service planning and critical intervention services.
This position is primarily responsible for the provision of clinical social work services to residents and families within residential long-term care and personal care home facilities and at time community-based clients. This position will also have some responsibility for the completion of RAI assessments for placement and other community support services. As part of an interdisciplinary team, the social workers primary roles are to facilitate, coordinate and provide leadership in the admission process through a single entry system, facilitate the resident's transition and psychosocial adjustment to residential placement, provide individual and group counselling, conduct bio-psychosocial assessment, facilitate palliative care, and assist with discharge planning, The successful candidate will also liaise with internal and external partners, monitoring/ licensing of personal care homes, and undertake workload measurement and reporting. Working directly with residents, families and care givers, the Social Worker II will continually assess and monitor resident strengths, resources, needs, and suitability for placement.
The successful candidate will adhere to standards for client services in accordance with the Canadian Association of Social Workers Code of Ethics, the Standards of Practice in Social Work, and the Social Workers Act of Newfoundland and Labrador. The successful candidate will be committed to partner with all team members to improve worker and client/resident safety by integrating safety into work practices, identifying risks, and implementing improvements.
Required qualifications:
• Undergraduate Degree in Social Work.
• Current registration with NLCSW.
• Familiarity with the Rai-Home Care and MDS Assessment tools.
• Considerable knowledge of the needs, resources and services available to seniors and individuals residing in facility-based care setting.
• Demonstrated ability in comprehensive assessment, interviewing and crisis intervention techniques, and social work programming and delivery methods.
• Established analytical capability and oral / written communication skills.
• Ability to work both independently and as part of an interdisciplinary team.

• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory certificate of conduct from the RCMP or local police authority.
Preferred qualifications:
• 2 years' experience within the last 5 years working in Long Term Care, Community Supports Services, and or acute care program areas.
• Graduate Degree in Social Work.
