Demands, duties, qualifications
Job Summary
This position is responsible for the provision of comprehensive social work services to Long Term Care residents and clients at various sites across the Bonavista, Clarenville and Burin Peninsula area. Specific duties include: assessment, counselling and coordination of admission/discharge, consultation, and assisting in the development of special programs. The Social Worker will also direct, supervise and mentor social work students. In performing these duties, the Social Worker will work as part of an interdisciplinary team.

This position is also responsible for the provision of professional social work services to acute care patients and clients in the Emergency departments at various acute care sites in Bonavista, Clarenville and the Burin Peninsula. As an integral member of the interdisciplinary team the social worker independently screens and assesses clients, formulates treatment and discharge plans and provides follow-up and aftercare. The goal of social work services is to improve health outcomes by providing services to individuals, families and groups and maximize their use of health and community services.

The Social Worker will also direct, supervise and mentor social work students.

This position reports to the Regional Manager of Allied Health and maintains accountability to the Social Work Professional Practice Consultant for integrity and professional practice issues.

Job Demands

Work demands do not result in considerable fatigue requiring periods of rest.
Occasionally, there is lifting or physical strength required (i.e. lifting or moving objects more than 50 lbs. such as moving clients in wheelchairs, helping clients with personal needs and carrying equipment).
Regularly sits to consult with clients, conduct therapeutic sessions, and to work on the computer.
Occasionally works in awkward or cramped body positions.
Pushes, pulls and sets up furniture for group work.
Fine finger/precision work is required for computer work.
Visual concentration is required to observe facial and body expressions/language, behavior, physical appearance in order to interpret, integrate these observations into meaningful information. It is also required when reviewing client files or notes and for documenting work.
Auditory concentration is required to listen attentively to the client and family members, to be alert to obtain and interpret information correctly, and to listen for tone, voice and the level of volume in their communications in order to detect distress/urgency.
A higher than normal level of attentiveness and carefulness is required when assessing clients, especially those who may be at a high risk, in order to ensure safety for all.
Lack of control over the work pace, and time pressures can occur. It is often challenging to balance client and non-client responsibilities.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador College of Social Workers (NLCSW) is required.
Minimum of three years of experience within the last five working as a Social Worker in an acute care or long term care program is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of discharge planning and community-based resources.
Knowledge of palliative and end of life issues.
Knowledge of case management.
Knowledge of crisis interventions and supportive counseling.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated ability to work with a high degree of independence and initiative.
Demonstrated strong program development and evaluation skills.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated ability to work effectively within an interprofessional environment and establish and maintain effective working relationships.
Demonstrated strong awareness of ethical principles and standards of professional conduct and practice.
Use of a personal vehicle is a condition of employment.
A satisfactory record of work performance is required.
