Demands, duties, qualifications The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client's needs. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers.
This position will support the FCT for the Baie Verte and Springdale areas. This position will work from the Springdale and Baie Verte sites as needed and will also support the Family Care Teams around the Central Zone.

Definition of work:
The Clinical Pharmacist IE will report to the Provincial Pharmacy Manager, Community Services and will work closely with the Central Zone pharmacy leadership team. While working to full scope and knowledge, the Clinical Pharmacist IE will provide proactive direct patient care and be responsible for providing pharmaceutical care and counseling to individuals and patient groups. The successful candidate will also provide comprehensive medication assessments to identify, prevent and/or resolve drug therapy problems, support optimal medication therapy for all patients, collaborate with health professionals in the acute care and community setting to support the patient's medication needs and coordination of medication access. The clinical pharmacist IE will provide orientation of other health professional learners to pharmacy, determine medication information needs of the Family Care Team Patients and develop and provide education as required and other required duties as required.

The successful candidate must be able to develop and maintain strong professional relationships with all members of the Primary Health Care Team and have strong clinical/ therapeutic skills to serve as the pharmacy consultant in this practice area. He/she must be able to work independently and be willing to assist in developing and maintaining medication-related policies and procedures to support safe medication use in line with regulatory requirements, accreditation standards and best practices.

This position may also require travel and the utilization of virtual technologies in supporting the clientele within the geographic area. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care; demonstrate a commitment to continuous quality improvement and demonstrate a commitment to safety partnering with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
• Bachelor of Science (Pharmacy).
• Successful completion of Pharmacy Examining Board of Canada National Exam.
• Current registration and licensure with the College of Pharmacy of Newfoundland and Labrador.
• Authorized by College of Pharmacy of Newfoundland and Labrador to prescribe (within scope of practice); to provide Opioid Agonist Maintenance Therapy; and to serve as a preceptor to pharmacy students.
• Demonstrated strong communication and presentation skills as well as an ability to work effectively and independently in a collaborative and multidisciplinary environment.
• Demonstrated critical thinking, problem solving and interpersonal skills.
• Demonstrated ability to integrate and evaluate pertinent data to problem-solve and make decisions effectively.
• Demonstrated ability to work effectively within an interdisciplinary environment and provide clinical consultation.
• Demonstrated effective communication, both verbally and in writing with co-workers, patients and their families.
• Exhibited ability to deliver/facilitate effective presentations.
• Possessed proven analytical, collaboration and organizational skills.
• Proven ability to assume professional responsibility for safe, ethical and effective practice.
• Consistent demonstration of NL Health Services core values (I CARE) Innovation, Compassion, Accountability, Respect and Excellence.
• A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory certificate of conduct from the RCMP or local police authority.

Preferred qualifications:
• Previous experience with a Regional Health Authority.
• Authorization by NLPB to administer medications by injection or inhalation
• Doctorate of Pharmacy
• Completion of a Hospital residency program
