We are searching for a permanent full-time Resident and Family Service Coordinator (Licensed Practical Nurse) to join our Parkland Station Yards team based in Moncton, New Brunswick.

This exciting new opportunity will be supporting the current nursing homes, Gordon Hall and Ocean Hall as well as the opening of Garraway Hall, a new 60-bed licensed long-term care facility located at Parkland Station Yards and opening in April 2025.

About the Opportunity

This position will be responsible to coordinate admissions and be a liason with residents and families. See below for details about the role:

  • Participates as a member of the interdisciplinary team, resident and family meetings, coordinating with other team members in assisting individual residents to meet their assessed needs. Provides support to residents and families as appropriate.
  • Collaborates with residents and family/friends to meet their program expectations and needs.
  • Conducts tours of the facilities and interviews prospective residents and/or families through home, hospital and/or office visits to obtain pre-admission information; answers inquiries from family members and general public on admission procedures; screens and assesses resident applications for admission.
  • Coordinates admission, transfer and discharge of residents in coordination with the interdisciplinary team.
  • Prepares funding requests and reports relating to resident needs; maintains resident records as required.
  • Supports resident and family with financial questions and concerns; investigates and rectifies any lapses in payment.
  • Develops and maintains relationships with the Department of Social Development, community institutions and community groups as necessary.
  • Prepares reports, records, statistics and other data as needed or requested.
  • Demonstrates effective communication strategies and skills when relating to residents, family/friends and team members.
  • Modifies service delivery to support the changing health status or responsive behaviors of the residents.
  • Promotes a safety culture (or acts as a safety role model) by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace, including but not limited to, providing supervision, training, information and equipment needed for employees to do their job safely.
  • Ensures resident safety by following and applying all standard operational procedures, safe work practices and training provided.

About You

In addition to placing high value on continuous improvement, collaboration and accountability, you bring:

  • Active registration with the Association of New Brunswick Licensed Practical Nurses (ANBLPN);
  • Ability to provide a clear criminal record with vulnerable sector screening upon hire;
  • Advanced proficiency in the English language is required, proficiency in French considered an asset;
  • Any previous course in Alzheimer's and Dementia Care to be a huge asset;
  • Previous working experience with elderly in long-term care or a senior living environment an asset;
  • A passion for the healthcare sector and/or ensuring seniors have access to quality accommodations, services and care
