Demands, duties, qualifications

Job Summary
This position reports to the Director of Primary Health Care and Chronic Disease Prevention and Management and is accountable for the provision of efficient, effective and quality Primary Health Care and Chronic Disease Prevention and Management serviceswithin the defined geographic area of Carbonear and the surrounding area of Eastern Rural Zone of NL Health Services.

This is an integral leadership team member supporting the program to deliver the full continuum of primary health care services. The position functions with a high degree of autonomy assisting the Director to implement key strategic directions, operational plans, objectives and strategies. The position develops, coordinates, integrates and assists in the development of interdisciplinary programs; monitors performance standards and existing practices, and designs new systems to improve organizational performance and staff competence. The position also promotes the integration and standardization of evidence informed practices; assists in the design of safe, effective and efficient quality care delivery systems; leads the development, implementation and evaluation of program specific projects; and supports clinical program priorities in alignment with strategic goals of NL Health Services.

The Manager coordinates a systematic, standardized Primary Health Care approach in NL Health Services' Primary Health Care clinic sites using the five principles of Primary Health Care which include access, continuity, person-focused, collaborative and team-based and engaged communities thereby improving quality of care in an effective, efficient and equitable manner.

The Manager is responsible for the program oversight of educational requirements of clinical staff, while providing direct supervisory, administrative and operational direction to the Clinical Educator and additional team members responsible for of the quality improvement and change management initiatives within the program.

Job Qualifications

Education
Bachelor's degree in a health-related discipline or social science.
Master's degree is preferred.

Experience
A minimum of five (5) years' experience within the last seven in progressive leadership roles is required.
Experience with project management an asset
Management experience is preferred.

Competencies (Knowledge, Skills & Abilities)
Comprehensive knowledge of community health programs, community health nursing, population health, particularly in the areas of primary health care and chronic disease prevention and management, interprofessional models of care, population health and change management.
Demonstrates exceptional oral and written communication skills including high level presentation skills for the purpose of facilitating decision making through clear and well-structured reports, presentations, briefs, submission sand strategic documents.
Demonstrates knowledge and skills in pertinent clinical area, and awareness and knowledge of organizational systems.
Establishes and adheres to clinical practice guidelines and organizational policies and procedures, service delivery standards and continually monitors and adjusts based on best available information.
Possess leadership experience in team building, program planning, evaluation and community development.

Other
A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.
