Demands, duties, qualifications DUTIES: The Nurse Practitioner (NP) is responsible for providing geriatric care to clients and families in the community environment and is a member of the multidisciplinary team with primary responsibility for the Home Dementia Care Program. This program is designed to provide enhanced support to individuals living in the community with moderate to late-stage dementia and their families. The successful candidate will complete community-based comprehensive geriatric assessments and support/implement strategies to address frailty, reduce falls, prevent delirium, and maintain optimal chronic disease management in the home. The NP will be required to select approaches of care and technology based on the client's needs and tailored to the client's social, economic, and cultural resources. The focus will be a Home First philosophy of care to maintain clients in their own home (inclusive of Personal Care homes) and to prevent unnecessary hospitalization. The successful candidate will function in the roles of practitioner, consultant, educator, leader, advocate, and researcher in close affiliation with providers supporting dementia care. Duties of the NP within the Home Dementia Care Program include integrating advanced knowledge and experience in delivering safe, effective, quality care to older adults with dementia in their home environment.

REQUIRED QUALIFICATIONS: Current registration with the Association of Registered Nurses of Newfoundland and Labrador, graduation from an approved School of Nursing and successful completion of the Nurse Practitioner Program are required. The successful applicant must be able to demonstrate an acceptable level of competency in clinical leadership skills, communication and organizational skills and be able to practice both independently and in collaboration with the interdisciplinary team. The successful candidate should possess considerable clinical experience in the care of the older adult population with advanced and current knowledge in comprehensive geriatric assessments, dementia assessments and proficient knowledge in differential diagnosis of dementia, delirium, and depression. Certification with the Canadian Nurses' Association in Gerontological Nursing is preferred. The incumbent must demonstrate strong written and oral communication skills. The ability to establish and maintain effective working relationships is essential. Minimum of five years of clinical experience out of the last seven years and two years clinical experience working in gerontological or psychogeriatric setting is considered an asset.
