Demands, duties, qualifications
Reporting to the Manager of Mental Health and Addictions Services, this position demands a high level of independence with work assignments of a complex nature and the use of specialized skills in the treatment of mental illness and addictions within an interdisciplinary team. Qualifications :
- Bachelor's Degree in social work
- Currently registered with NLCSW
- 2 years current experience (within the last 5 years) in an addiction, and/or mental health setting
- Advanced clinical knowledge and skills in the areas of assessment, intervention, case planning and community engagement
- Specific knowledge and experience in the areas of mental illness, addictions, and its impacts on individuals and families
- Demonstrated ability in interviewing and crisis intervention techniques
- Demonstrated leadership ability in the areas of interdisciplinary collaboration, advocacy, mentorship and program planning
- Demonstrated analytical, as well as oral and written communication skills
- Demonstrated independence, initiative, and ability to work with others
- Willing to work a flexible work schedule, may include evenings and weekends
- Use of a private vehicle and valid driver license are a condition of employment
- Satisfactory Record of work performance and attendance is required for this position
- must be eligible for registration with the Newfoundland and Labrador Association of Social Workers.

Duties :
The FACT Coordinator functions in a leadership role within the interdisciplinary Flexible Assertive Community Treatment (FACT) team with duties including:
- referral management
- liaising with referral agents
- chairing daily FACT meetings
- coordinating the daily schedule
- coordination of Psychiatry clinics and/or telehealth sessions
- consultation and collaboration with internal and external service providers and family
- coordination and monitoring of treatment plans and service delivery
- will be responsible to provide clinical leadership to other team members
- will assist with program planning and development, evaluation and change management
- While the primary function of this position is to support operations of the FACT Team, the Social Worker II will be involved in assessment, treatment, follow up and support of individuals with mental health and addictions needs throughout various mental health and addictions programs and services.
- Other related duties as required; travel may be required; may be required to work alone.
