Job Summary:
We are seeking a dedicated and compassionate Social Worker to join our team at the Northside Community Guest Home, a long-term care facility committed to enhancing the lives of our residents. In this role, you will provide essential psychosocial support, advocacy, and counseling to residents and their families. You will play a key part in helping residents adjust to their new environment, addressing emotional and social needs, and fostering a supportive, respectful community. As a vital member of our multidisciplinary healthcare team, you will collaborate closely with medical staff, develop care plans, and advocate for resident well-being to ensure the highest quality of life.
Responsibilities:
- Conduct preadmission assessments to evaluate the needs of potential residents.
- Assist in the admission procedure by advising on pre-admission assessment findings.
- Provide psychosocial support to residents and their families.
- Advise residents and families regarding moves within the Home and coordinate notifications to cable and telephone companies.
- Collaborate with doctors, therapists, and other staff members to ensure seamless care.
- Offer counseling services to residents, addressing their emotional and psychological needs.
- Maintain comprehensive resident files, preparing verbal and written reports and correspondence related to your work.
- Collaborate with other disciplines to diagnose problems, formulate treatment plans, and evaluate progress.
- Help residents adjust to long-term care living, assess their need for further care, and act as an advocate for their needs.
- Participate in continuing education activities and provide in-service training to other staff members.
- Provide field instruction for graduate-level Social Work students.
- Establish and maintain sustainable working relationships with community agencies, the Department of Health, and the district health authority.
- Obtain cost approvals for resident items funded under the long-term care policy.
- Develop and update departmental policies, programs, and procedures related to Social Work within the Home.
- Undertake additional duties and responsibilities as directed by the Administrator.
- Chair multi-disciplinary meetings to facilitate collaboration and coordination of care.
- Perform charting duties accurately and efficiently.
Qualifications
- Master's or bachelor's degree in social work from an accredited school of Social Work.
- Previous experience in a long-term care setting.
- Registration with the NS Association of Social Workers.
- Ability to work independently and collaboratively as part of a healthcare team.
- Proficient in social work intervention methods, particularly within a long-term care setting.
- Knowledge of laws and regulations pertaining to Homes for Special Care.
- Minimum of two years' experience as a Social Worker, preferably in a long-term care facility, hospital, or similar setting
