Our team is growing! North Queens Nursing Home is currently accepting applications for a part-time Care Navigator. Reporting to the Director of Care, the Care Navigator is responsible for organizing admissions, transfers and discharges of residents, for establishing and maintaining harmonious relations with residents and their families and is responsible for collaborating with the residents and their families to ensure their care needs are met. The Care Navigator is also responsible for booking, coordinating and communicating medical appointments for the residents.
Qualifications:
1. Relevant education suitable for the position. Related experience will be considered equivalent to education.
2. Working knowledge and understanding of the care needs of the residents - previous experience in a long-term care facility is an asset.
3. Excellent communication skills, both verbal and written.
4. Ability to establish and maintain effective harmonious relationships with staff, residents, families, and other stakeholders.
5. Strong computer skills, including Microsoft office.
6. Demonstrated problem solving skills.
7. Commitment to a high degree of confidentiality.
8. Must be able to work independently with minimal supervision.
9. Regular attendance in current and previous employment.
Hours of Work: 32 hours per week
Rate: $22-26/hour, depending on qualifications, with competitive benefits
