Demands, duties, qualifications

Definition of Work: Western Health is currently looking for a Licensed Practical Nurse who will work as part of a Family Care Team, supporting clients across health care services in a health navigator role. Working in collaboration with the individual and/or family and the interdisciplinary team to assist the individual/family in developing skills, accessing support for and addressing health and social needs. The LPN will provide consistent and ongoing care and support including coordinating services with other team members and community resources adjusted to meet the changing needs of the individual.

In addition, and in accordance with established policies, procedures, best practice standards and guidelines, the LPN will be responsible for performing specific assigned nursing procedures and services to address the needs of individuals with a variety of complex chronic conditions. The LPN will be responsible for the implementation of the BETTER program in the role of a BETTER Prevention Practitioner providing outreach to clinics throughout the Health Neighborhood. The candidate will coordinate integrated care delivery for selected populations to improve clinical outcomes through the implementation of health and disease management programs. The LPN will also function as a core member of the interdisciplinary care team and may fulfill several roles and responsibilities within their scope of practice. Mileage and appropriate per diems will be paid in accordance with the CUPE Collective agreement.

Required Qualifications: Graduation from an approved practical nursing program and current registration with the College of Licensed Practical Nurses of Newfoundland and Labrador are required.
