Definition of work:
This is an advanced professional level position involving a high degree of responsibility and autonomy that requires outstanding communication and clinical skills. Reporting to the Regional Manager of Community Support Services (CSS), the Acute Care Social Worker is an integral member of the CSS division and supports safe and effective transitions of care for patients through the completion of specialized clinical assessments.
The Acute Care Social Worker accepts and screens referrals from all units through a case management model of service delivery. The successful candidate is responsible for interventions which facilitate adjustment to the impact of illness, treatment and hospitalization. The clinician assesses whether supportive services are required for discharge and coordinates community follow-up where appropriate. The successful candidate actively incorporates the components of informed decision-making and principles of person-and-family-centered care within their daily practice. The Acute Care Social Worker's assessment recognizes the importance of understanding the patient's expectations and wishes relative to the provision of healthcare and in the context of available options.
The successful candidate is an integral member of an interdisciplinary healthcare team. The Social Worker understands and demonstrates the importance of strong communication and collaboration. The successful candidate will participate in team huddles, interdisciplinary rounds and family meetings as required/appropriate.
The Acute Care Social Worker is trained in the clinical tools required to assess supportive services within the Community Support Services Division and demonstrates extensive knowledge of the needs of the patient population and available community resources. The successful candidate supports the home first philosophy of care and appropriate utilization of acute care beds.
The Social Worker respects the inherent rights and dignity of individuals. They adhere to the Canadian Association of Social Workers Code of Ethics, Standards of Practice, and the Social Workers Act of Newfoundland and Labrador. The successful candidate
must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centred care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
• Bachelor Degree in Social Work.
• Current registration with NLCSW.
• 2 years' experience within the last 5 years working in the Community Supports program.
• Familiarity with the Provincial Home Support Program and the Long Term Care and Community Support Program Adult Needs Assessment.
• Considerable knowledge of the needs, resources and services available to seniors and individuals with disability.
• Demonstrated ability in comprehensive assessment, interviewing and crisis intervention techniques, and social work programming and delivery methods.
• Demonstrated analysis skills and the ability to maintain effective working relationships.
• Established analytical capability and oral/written communication skills.
• Ability to work both independently and as part of a team.
• Consistent demonstration of Central Health's corporate values; Innovation Compassion, Accountability, Respect and Excellence (I CARE)
• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory Certificate of Conduct from the RCMP or local police authority.
Preferred qualifications:
• Experience in utilizing RAI-HC clinical tool.
