Demands, duties, qualifications
Definition of Work: Reporting to the manager of Primary Health Care and Chronic Disease, this regional position will work within a population health and primary health care framework. Responsibilities include supporting, coordinating, implementing, and evaluating the chronic disease self-management programs, including Improving Health My Way, BETTER and Health Coach programs as well as, other self-management support strategies for the population served by Central Health. This position also provides professional development opportunities to increase the knowledge and skill of health service providers and key stakeholders in self-management support. The coordinator will participate on internal and external committees as required, as well as, develop and maintain effective relationships with appropriate program partners. The successful candidate will be committed to safety and partner with all team members to improve worker and client/patient safety by integrating safety into work practices, identifying risks, and implementing improvements.
Responsibilities:
• Coordinate and evaluate the chronic disease self management programs throughout the region
• Provide leadership to Improving Health My Way program leaders
• Lead Communities of Practice to support the leaders in their knowledge and skills in the BETTER and Health Coach programs
• Assess, develop, and evaluate interdisciplinary staff education needs related to self-management and self-management support strategies
• Link with Primary Health Care teams and other key stakeholders to facilitate community and program engagement
• Travel throughout the central zone is required
Required Qualifications:
• Graduation from an approved university with a degree in health or social science
• Demonstrated knowledge of program planning, development, and evaluation; community mobilization; adult learning principles and chronic disease prevention and management
• Demonstrated ability to be self-directed and work independently with limited supervision while working collaboratively within an interdisciplinary team
• Demonstrated commitment to Professional Development and Continuing
Education
• Demonstrated ability to establish and maintain effective working relationships
• Demonstrated experience in working with a wide variety of health partners, resource development, and community mobilization
• Demonstrated effective organizational, interpersonal, and decision making , as well as, written and oral communication skills
• Demonstrated knowledge of computer based applications such as Microsoft Word, Excel, Publisher, etc.
• A valid Newfoundland and Labrador Class 5 Driver's license and use of a private vehicle
• A satisfactory record of work performance and attendance is required for this position
• A satisfactory certificate of conduct from the RCMP or local police authority
