Demands, duties, qualifications The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client's need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers. The FCT for the Brookfield/ Centreville will include the geographical areas of Brookfield and Centreville.

Definition of work:
This is an advanced professional level position involving a high degree of responsibility, autonomy and independence. The position involves planning, coordinating, and evaluating the community based occupational therapy program. The successful candidate will provide occupational therapy services as part of an Interdisciplinary team with the goal of supporting individuals to live safely and effectively in the community. The successful candidate will perform short-term interventions such as equipment prescription, development of home programs, environmental modification, and client/caregiver education. These interventions enable clients to achieve the highest and safest level of occupational performance possible within their home environment to avoid hospitalization. The Occupational Therapist II will ensure person and family centered care that wraps available resources around the individual wherever the individual resides. The Occupational Therapist II will establish a preventative, integrated, person and family centered approach driven by individualized assessment of needs and client individual choice to promote aging in place. This position requires a good understanding of primary health care, as well as, the ability to develop and maintain strong linkages with internal and external partners. Travel throughout the region may be required.

The successful candidate will be committed to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
• Graduate degree specializing in Occupational Therapy from an approved university.
• Certification with the Canadian Association of Occupational Therapy (CAOT).
• Licensure with the Newfoundland and Labrador Occupational Therapy Board (NLOTB).
• 2 years work experience in the field of occupational therapy.
• Experience and knowledge in areas of community, policy, and program development, as well as, program evaluation.
• Demonstrated research, analytical, communication, and interpersonal skills.
• Willing to work a flexible work schedule.
• A valid Newfoundland and Labrador Class V Driver's license.
• Consistent demonstration of Central Health's corporate values; Compassion, Accountability, Respect, Stewardship and Quality
• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory certificate of conduct from the RCMP or local police authority.
