Hours of work will follow the flexible work schedule in accordance with Article 19.01(a)of the NAPE-HP Collective Agreement, based on 75 hours bi-weekly (by mutual agreement). Variable shifts may apply.
Salary CGHP-39 of the NAPE-HP salary scales (CAD40.07 - CAD48.73 per hour) plus the applicable Labrador Allowance

Demands, duties, qualifications
This position reports to the Manager of Community Supports and clinically supervised by the Social Worker III - Interprofessional Practice, the primary purpose of this position is to assist in the provision of social work services to the clients within the areas served by the Labrador Health Center. The primary focus of this position is the provision of services to clients under Acute Care. The primary roles include clinician, facilitator, coordinator, and consultant.
Qualifications
:
- High School Diploma or equivalent and successful completion of a Bachelor of Social Work degree;
- Preference will be given to individuals with 2 years' experience in health care;
- Current registration with the Newfoundland and Labrador College of Social Workers;
- Evidence of ongoing professional development;
- Willingness to travel within the region;
- Demonstrated effective clinical social work practice;
- Ability to work with minimal supervision;
- Knowledge of health care policies and trends;
- Basic computer skills training is preferred;
- The successful applicant will be required to submit a satisfactory certificate of conduct.
Duties and Responsibilities:
- Screens, identifies and prioritizes referrals based on complexity and high-risk factors to identify clients for social work intervention;
- Conducts assessment interviews and admission procedures with individual and family to assess the nature and extent of services required;
- Develops individuals' treatment plans and programs through consultation with client, family and other team members;
- Provides direct counselling to clients and families;
- Attends unit/service rounds with the inter-disciplinary health care team;
- Participates as a team member in the Regional Assessment and Placement Committee;
- Assists with the discharge planning process within the Labrador Health centre;
- Facilitates multi-disciplinary participation in the discharge planning process;
- Facilitates the safety of all clients and staff within the social work treatment area/department;
- Ensures compliance with all safety precautions/regulations;
- Ensures statistics are kept in accordance with up-to-date provincial MIS standards;
- Ensures compliance with all policies around safety issues/concerns - i.e., incident reporting, defensive driving manual;
- Commits to self-directed continuous learning/education;
- Other related duties.
