Through a new, innovative partnership with Nova Scotia Health (NSH), we are opening the first transitional health facility in our province, in West Bedford. Shannex is the operating partner for the transitional health facility on behalf of Nova Scotia Health. you will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services before they can safely return home/community.
We are searching for a Physiotherapist to join our Transitional Health Facility located in West Bedford, Nova Scotia.
The transitional health model supports NSH patients/families who require additional time, services, and support to transition back to home/community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation. Patient specific care focuses on maintaining and/or improving functional abilities and health status in a positive environment where the Physiotherapist encourages adherence to treatment plans and preventative care guiding patients/families with their knowledge and expertise.
The Physiotherapist is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.
The Physiotherapist supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

About the Opportunity
- Assesses, diagnoses, and treats patients in a tertiary care setting who present with actual or potential impairment, pain, functional limitations or disabilities. Physiotherapy assessment involves obtaining a detailed medical history, performing specific and standardized tests and evaluation procedures to determine a physical diagnosis.
- Encouraging, motivating, and assisting patients with hygiene and daily living activities, such as: bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient's care plan
- Plans intervention strategies by promoting active involvement of the patient, family, or caregiver in establishing physiotherapy goals which are based on assessment findings, prognosis, and evidence-based practice. Selected interventions are implemented to achieve and maintain optimal health fitness and function, encourage self-management, manager identified impairments and disabilities and relieve or manager pain.
- Participates in interdisciplinary rounds, team meetings and family meetings regarding patient care, prognosis, and discharge, liaises with medical staff concerning patients' progress and needs. A physiotherapist maintains and advances current knowledge and skills through ongoing continuing education.
- Develops individualized treatment plans based on clinical records, observation, or referral; confers with interdisciplinary staff regarding an overall treatment plan for patients.
- Maintains an inventory of physiotherapy equipment and supplies.
- Ensures patient safety by following and applying all standard operational procedures, safe work practices and training provided.
About You
- A master's degree in Physiotherapy
- Current registration with the provincial College of Physiotherapists
- Minimum of two years clinical experience
- Knowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy and their application in specific cases.
- Knowledge of the potential hazards involved in physiotherapy treatments and precautionary measures to be taken.
- Demonstrated ability to exhibit high standards of ethical conduct, honesty and integrity.
- Ability to communicate effectively and efficiently, both verbally and in writing.
- Expresses information to individuals or groups effectively, taking into account the audience and the nature of the information. Listens to others and responds appropriately.
- Knowledge of skeletal, muscular and neural anatomy and physiology as it applies to physiotherapy.
