We are searching for a Quality and Education Coordinator to join our Transitional Care team based in Bedford, Nova Scotia.
Shannex aims to build capacity in our health system by improving access and flow and effective resource utilization. The Goal of Transitional Care is to provide care that focuses on the patient's functional ability and support network, providing the necessary support and resources to transition back to community, either returning to their prior living disposition or another care setting. In partnership with Nova Scotia Health (NSH), we are excited to open and operate the first Transitional Care Centre in Nova Scotia.
Reporting to the Director, Clinical Services, the Quality and Education Coordinator, Transitional Care works collaboratively with the Transitional Care Team, Shannex's Clinical Services Team, and members of the NSH Team, to provide quality patient care consistent with applicable legislation, Accreditation standards, evidence-based practices, and policies and procedures.

About the Opportunity
- Collaborates with the Transitional Care Team to support and care for persons and their support network when encountering transitions within and across settings, including (but not limited to) primary care, home or community care, mental health and substance use health settings, acute care, rehabilitation, long-term care (LTC), and shelters.
- Researches and provides evidence-based recommendations for nurses and members of the Transitional Care Team to support safe and effective transitions in care for adults (18 years and older) and their support network.
- Works closely with Shannex and NSH to identify clinical education needs for employees and supports with developing a clinical education plan for roll out.
- Delivers clinical and IPAC training sessions, workshops, and educational programs and monitors effectiveness.
- Participates in development, evaluation and revision of clinical policies, procedures, and education consistent with legislation, best practices, and Accreditation standards.
- Participates in interprovincial committees/working groups and shares and disseminates knowledge with the Clinical Services Team and broadly with Shannex and NSH, as requested.
- Leads quality improvement initiatives.
- Presents data findings and quality improvement project outcomes, as requested.
- Participates in Accreditation preparations.
- Conducts regular clinical and IPAC audits to determine compliance with regulatory requirements, clinical policies and procedures, and organizational standards, and communicates results in a report to the Transitional Care Team and the Director, Clinical Services in a timely manner.
- Collaborates with the Transitional Care Team and the Director, Clinical Services to create, monitor, and evaluate Quality Action Plans, where required.
- Monitors and analyzes findings and trends related to regulatory requirements, quality indicators, and clinical outcomes and identifies opportunities for improvement.
- Participates in the implementation and surveillance of the IPAC Program.
About You
In addition to placing high value on continuous quality improvement, collaboration and accountability, you bring:
- Bachelor of Nursing, or Bachelor of Science in Nursing
- Current registration or eligibility to be registered with appropriate nursing regulatory body
- Minimum five (5) years nursing experience which includes clinical, administrative, and leadership experience
- Experience using clinical outcomes and data to evaluate and guide clinical performance activities
Education and experience considered an asset
- Master of Nursing, Master of Science in Nursing, or another healthcare-related post-graduate program
- Completion of post-graduate clinical education and/or certifications (i.e., Restorative Care, Gerontology, Palliative and End-of-Life Care, Behavioural Supports, Skin & Wound Care, Infection Prevention & Control [IPAC])
- Certified in Rehabilitation Nursing (C)anada and Gerontological Nurse Certified (C)anada through the Canadian Nurses Association
- Bilingualism in English and French
