Job Summary
The Social Worker II provides professional, comprehensive and clinical leadership (sole charge) social work duties within the Caribou Memorial Veterans Pavilion. The focus of the Social Worker II position includes intake/assessment and placement, case management, case planning, individual and group counselling, discharge planning, coordination with other service providers and the provision of education, consultation, guidance and counselling to clients, the general public and external agencies. This is a highly responsible social work position with a long term care facility providing services to a unique population. This position involved consulting and advising other team members in areas of clinical expertise, participating in the orientation of new hires, program evaluation, quality initiatives and strategic planning activities, ensuring compliance with professional standards and professional development. Work is performed with considerable independent judgement, and initiative within the scope of policies, regulations and legislated authority. Accountability is established through clinical documentation, documentation review processes, self/peer review and performance appraisal. The Social Worker II will be required to provide field supervision to Social Work students. This position will work in collaboration with all members of the Interdisciplinary Care Team to achieve operational and resident-centered care objectives.

This position reports to the Administrator, Resident Care and Site Manager of the Caribou Memorial Veterans Pavilion and maintains accountability to the Social Work Professional Practice Consultant for professional integrity and professional practice issues.

Job Demands

Occasionally, there is lifting or physical strength required to lift or carry objects up to 10 lbs.
Regularly sits to consult with clients, conduct therapeutic sessions, and to work on the computer.
Occasionally works in awkward or cramped body positions; pushes, pulls, and sets up furniture for group work, and drives to conduct home visits with clients.
Regularly uses fine finger/precision work on a computer.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador College of Social Workers (NLCSW) is required.
Minimum of three years of experience within the last five working as a Social Worker in an acute care, long term care or community health care setting is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, and Medical Assistance in Dying.
Knowledge of community-based resources, systems and programs.
Knowledge of palliative and end of life issues.
Knowledge of crisis interventions and supportive counseling.
Knowledge of case management.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated ability to work with a high degree of independence and initiative.
Demonstrated strong program development and evaluation skills.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated ability to work effectively within an interprofessional environment, and establish and maintain effective working relationships.
Demonstrated strong awareness of ethical principles and standards of professional conduct and practice.
A satisfactory record of work performance is required.
