Employment Status : Temporary Full-Time (12 months)

Salary Band : $ 47,922 - $ 59,903 / year

In this role, you will :

  • Create knowledge transfer and learning strategy using educational tools to provide a learning path that develops understanding of social prescribing in community.
  • Provide on-site facilitation of presentations and workshops to community in a service coordination hub that are aligned to learning strategy.
  • Develop relationships within healthcare that have potential to ingrain social prescribing in their practice.
  • Identify best path forward and co-develop community ownership strategy with Project Manager.
  • Working with a specialized network of social prescribing experts, standardize social prescribing knowledge through the development of a portable training toolkit that will be utilized by community organizations to expand social prescribing within their services.
  • Working with a specialized network of social prescribing experts, create a link worker competency framework to be shared to community.
  • Working with healthcare partners and appropriate stakeholders, create a financial assistance and support pathway and financial wellbeing toolkit for distribution to community organizations.
  • Conduct analysis and create final report on community needs and gaps as identified by the service coordination hub.
  • Working with the evaluation team and independently, conduct analysis and manage expectations on community-led approach using post-meeting feedback surveys, 1 to 1 interviews etc.

What we are looking for :

  • The minimum qualifications for this position are post secondary education and 3-5 years experience in healthcare and/or education.
  • Strong knowledge and ability in using business support processes and tools (e.g., Microsoft Office suite, SharePoint, databases and/or other tracking systems).
  • Superior organizational skills and keen attention to details and timelines are essential.
  • Resilience and adaptability to changing circumstances and an evolving environment.
  • Strong relationship building and customer service skills, demonstrated attention to client objectives and quality assurance.
  • Fluency in English; working proficiency in French an asset.
  • Strong written, verbal and interpersonal communication skills to coordinate a variety of event variables.
  • Experience with, curriculum or learning strategy development, social prescribing, coordinating community learning events, facilitation and training, or workshop delivery.

Working conditions :

  • Much of the work is performed in an office/hybrid work environment.
  • May include long to moderate hours sitting and using computers and office equipment.
  • May be required to do some light lifting of supplies and materials from time to time up to 20 kg (or another weight).
  • Events require a higher than usual physical effort during the preparation phase which includes transport of event materials (lifting and carrying boxes, setting up and dismantling displays).
  • This position will require travel to attend conferences, workshops and meetings as required.
