Job Description

Regional Manager - Family Care Team, Permanent Full TimeLewisporte Area, Central Zone, NL Health Services

Definition of work:
Reporting to the Regional Director - Primary Health Care, Community Nursing & IPAC, the Manager will collaborate in the development and integration of the Central Zone Primary Health Care (PHC) Framework. The Manager will support initiatives identified within Central Zone's Strategic Plan while continuing to strengthen partnerships with community stakeholders, focusing on Patient and Family Centered Care and access to care. Incorporating change management strategies, the Manager, in partnership with stakeholders such as the Shalloway Family Practice Network and Community Advisory Committees (CAC), will focus on establishing and managing the Family Care Team (FCT) in the Lewisporte area while leading strategies that continue to enhance the tenants of Primary Health Care, including:

2. Access & Continuity of Care
3. Inter-Professional Collaboration
4. Building Improvement & Transformation Capacity
5. Evaluation
The Manager will collaborate in the development, delivery and evaluation of priority system transformation initiatives while supporting team direction and oversight. The manager will promote change leadership and a culture of safety. This position will also be responsible for operational management of the Community Support staff.

Key Responsibilities:
* Development of Central Zone PHC FCT Framework
* Collaboration with PHC team members and stakeholders to develop and implement goals within the FCT
* In collaboration with the Director of PHC, Community Nursing & IPAC; Senior Director of PHC & Community Services, and the Medical Director Primary Care, develop a 3- and 5-year action plan towards PHC Reform
* Develop operational and clinical policy for Family Care Teams
* Ensure delivery of safe and quality clinical services and develop/implement standards of care
* Facilitate delivery of a communication plan with public and key stakeholders in the geographical area
* Collect data for analysis, evaluation, and reporting purposes
* Support and analyze community needs assessment in partnership with Manager - PHC & Chronic Disease
* Support and maintain electronic record keeping, integration and panel management
* Ensure a best practice approach to program development and service delivery
* Support clinical adoption and change management

Required qualifications:
* Undergraduate degree in a health-related discipline, system design, project management, interdisciplinary studies or public health.
* A minimum of 5 years within the last 7 of progressive leadership experience with a proven track record of system transformation and re-design.
* Experience in PHC management, health program planning, including community mobilization and health education.
* Experience in creating successful partnerships with programs and organizations; provincially, nationally or internationally.
* Experience mentoring others on the development of initiatives within complex system designs with patients and families as active partners.
* Consistent demonstration of NLHS Corporate Values; Innovation, Compassion, Accountability, Respect, and Excellence.
* Demonstrated commitment to person and family centered care.
* Exceptional oral and written communication skills.
* Effective time management skills and the ability to prioritize.
* Dynamic leadership style focused on engaging others, developing coalitions and achieving results.
* Proven negotiation and conflict resolution skills.
* Ability to function effectively in a high pressure, diverse and changing environment.
* Class 05 driver's license and the ability to travel.

Preferred qualifications:
* A graduate degree in a health-related discipline
* A satisfactory record of work performance and attendance is required for this position.
* A satisfactory certificate of conduct from the RCMP or local police authority.
* LEAN/Prosci certification considered an asset
* Knowledge and/or experience with electronic health solutions
* Experience working within rural health care service delivery models

Hours of work: 70 hours bi-weekly (Flexible schedule/hours of work will be required)
Salary: HL-21 ($72,639-$94,431)
