Registered Nurse IIC - Care Facilitator
Temporary Full-Time
Location: Happy Valley-Goose Bay, NL

About the Role

This position reports to the Regional Manager Renal Services and is responsible for the assessment, planning, teaching and supporting of clients with renal failure. The successful incumbent will be expected to work both independently and in collaboration with others to provide high quality nursing services that are based on Best Practice. Understanding and applying concepts of adult teaching and learning as well as significant leadership and organizational ability are required for this position. The incumbent is responsible for maintaining professional and organizational standards and practice in accordance with the LGH MoNCP. The successful applicant must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.
Qualifications:
" Graduation from an approved/recognized school of nursing program is required;
" Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
" Current Canadian Nurses Association Nephrology certification or willingness to complete.
" A minimum of two (2) years clinical nurse experience plus two (2) years hemodialysis experience;
" Excellent leadership, interpersonal, communication and organizational skills;
" Proficient in the use of basic computer programs including Meditech and Microsoft Office;
" Current CPR certification.
" The successful applicant will be required to submit a satisfactory certificate of conduct.
" Any combination of education and nursing experience may be considered.
" The successful applicant will be required to submit a satisfactory Certificate of Conduct and Vulnerable Sector Check;

Duties: (Included but not limited to)
" Organizing, planning, implementing and evaluating the plan of care for assigned clients;
" Participates in the auditing process as required;
" Documents as per professional and organizational standards;
" Administers medication and participates in Medication Reconciliation;
" Participates in the orientation, mentorship and preceptoring of staff;
" Represents nursing on various committees as required;
" Participates in quality assurance initiatives as requested/required;
" Practices independently according to professional and organizational standards;
" Acts as a liaison between the client and other healthcare providers;
" May be required to complete medevac's occasionally;
" On call/stand-by may be required;
" Other related duties as required.
" Assesses client knowledge of renal failure through interviews and observation to determine the individual client's educational needs.
" In consultation with the client and his/her family, develops and updates appropriate teaching and care plans according to the identified client needs and individualized goals.
" Data collection and interpretation as it relates to the Hemodialysis Program.
" Works closely with nephrologists, multidisciplinary renal team, patients and families to provide high quality care within Best Practice Guidelines.
" Attends telehealth clinic appointments with nephrologists.
" Monitors quality indicators to ensure treatment adequacy and patient safety is maintained for all hemodialysis clients
" Provides assessment, education, training and decision support for patients/family as it relates to hemodialysis treatment regime.
" Acts as liaison to within the NL Health Services and professionals regarding care (i.e. medevac, dialysis transplant list, line change, new or transient patients, vascular access etc.).
" Audit assignment, unit and patient communication boards
" Engages the interdisciplinary team in a collaborative approach to professional practice issues and acts in the role of Liaison between all members who participate in the care of the patient and their family.
" Responsible to assist with organization Medivacs
" Communicates and collaborates with hemodialysis vendors.
" Ensures equipment is functioning and available.
" Ensures education, policies and documentation are current and communicated
" Provides clinical competency support to dialysis nursing staff
" Responsible for providing feedback to Clinical Nurse Manager on staff performance
" Attends provincial committee meetings and participates in all other committees as required.
" Coordinates unit orientation for staff and acts as preceptor when required.
" Participates in research activities as related hemodialysis clients. .
" Travel will be required.
" Other related duties as required.

Salary:
" NS-32 of the RNUNL Salary Scales, $43.72 to $54.16 per hour, plus the applicable Labrador Allowance, and with a locum premium rate of an additional $25/hr for hours working in the locum position. Locums must agree to work a minimum of fourteen (14) continuous days for this benefit to apply. Other benefits in accordance with the RNUNL Collective Agreement.

Hours of Work:
" 75 hours Bi-weekly; Variable shifts may be required.
