Home Based Therapy Registered Nurse II, Permanent Full-TimeHemodialysis-Regional Renal Program, Central Newfoundland Regional Health Centre, Grand Falls-Windsor

Definition of work:
This is a professional nursing position with responsibility to plan, implement, and evaluate the Home-Based Therapy program for Central Health. In this role, specific duties include Pre-Renal Nurse developing objectives, policies, and procedures to achieve acceptable standards. will provide teaching and counseling to patients, families, and staff for pre-renal, peritoneal dialysis and home hemodialysis while assessing the patient's knowledge of renal failure through interviews and observation, to determine the individual patient's educational needs. The successful candidate will also be responsible for developing appropriate teaching plans according to the identified patient needs and individualized goals for health management management with the patient and his/her family to promote independence at home.

The successful candidate will act as a consultant/expert in home-based dialysis therapies and resource for all members of the multidisciplinary team.

This is a position of leadership in which Pre-Renal Nurse will work closely with and report directly to the Regional Nurse Manager, Renal Program. The successful candidate will also work closely with Nephrologists, the multidisciplinary renal team, patients and their families to provide high quality care in keeping with current Best Practice Guidelines. Travel within the region may be required. This position also encompasses work on various committees, as well as, statistics collection and interpretation and possible participation in kidney health research both locally and nationally. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements

Required qualifications:
* Bachelor's Degree in Nursing or willingness to complete.
* CNA certification, or must be willing to complete.
* Graduation from an accredited School of Nursing program.
* Current registration with the Association of Registered Nurses of Newfoundland and Labrador.
* A minimum of 2 years nursing experience in Hemodialysis in the past 5 years
* BCLS certification for health care providers from the Heart and Stroke Foundation.
* A valid Newfoundland and Labrador Class IV Driver's License and the use of a private vehicle are conditions of employment.
* Demonstrated ability to function autonomously with minimal supervision.
* Demonstrated ability to implement a problem-solving approach as necessary
* Consistent demonstration of Central Health's corporate values; Innovation, Compassion, Accountability, Respect, and Excellence (I CARE)
* A satisfactory record of work performance and attendance is required for this position.
* A satisfactory certificate of conduct from the local police authority.

Hours of work: 75 hours bi-weekly (shift work may be required)
Salary: NS -29 ($39.71-$49.10)
