Job Description

The Hospital Elder Life Program (HELP) is a comprehensive program of care for hospitalized older patients, designed to prevent delirium and functional decline. The program is intended to integrate the principles of geriatrics into standard nursing and medical care on any hospital unit and to bring geriatric expertise to bear on patient management on the HELP patients, and ultimately to patients throughout the organization.

The Elder Life Specialist (ELS) oversees the daily operations of the HELP. Daily operations include screening and enrolling patients, orienting patients and families to the program, and assigning appropriate intervention protocols. The ELS will develop and oversee implementation of appropriate volunteer assignments for patients and assume direct responsibility for the Daily Visitor, Therapeutic Activities, Early Mobilization and Feeding Assistance Programs. The ELS will assist with recruitment, screening, and placement of program volunteers. The ELS will assist with recruitment and screening of volunteers, be responsible for training and orientation, and provide supervision and retention of HELP volunteers. Additionally, the ELS will perform regular evaluations, monitor and record adherence to the program.

The HELP involves specially trained volunteers providing targeted interventions that address geriatric issues known to contribute to cognitive and functional decline during hospitalization. The program is not limited to the maintenance of patients physical and cognitive functioning throughout hospitalization; but, also to provide seamless coordination of care for the elderly throughout the hospital, to maximize independence at discharge and to prevent unplanned readmissions. The ELS will also collaborate with community resources and other hospitals and healthcare professionals, as required.

Qualifications
  • Bachelor's Degree in Human Services or Healthcare related field required
  • Master's degree preferred
  • Current registration in good standing with respective governing College
  • Certificate in Gerontology preferred
  • BCLS Certification required
  • Strong passion and desire to work with the elderly
  • Minimum 2 years' experience in a supervisory position
  • Minimum 2 years geriatric experience
  • Previous experience in program and training development
  • Experience in developing policies and procedures
  • Innovative, creative and professional role model
  • Demonstrated ability to prioritize/organize multiple demands
  • Ability to work autonomously and as a team member with peers, Physicians, and Management Team
  • Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint
  • Must be able to meet the physical demands of the position
  • Satisfactory employment and attendance record required

Additional Information

Hours: Monday to Friday - Days; 7.5 hour shifts; 08:30-16:30 (hours subject to change based on operational requirements)

Salary: $46.004 to $59.637 per hour
