Position: Social Worker/Substance Use Counsellor

Program Description:
The Emergency Treatment Fund (ETF) is a funding initiative offered by Health Canada to municipalities and Indigenous communities to address urgent substance use needs.
The HOME Team is a joint initiative between the Guelph Community Health Centre, the Sanguen Health Centre, and the Mount Forest Family Health Team, which will provide mobile wrap-around support services to individuals struggling with substance use who face barriers to accessing traditional services. The HOME team will expand on existing programs to operate two mobile outreach vehicles one in the City of Guelph, and one in Wellington County, and offer additional place-based supports.
Reporting to the Guelph CHC Allied Health Supervisor, the HOME Team Social Worker will provide substance use counselling and generalized counselling support to community members engaged with the HOME Team in Guelph and the County of Wellington. They will work with clients where they are at to support the creation and pursuit of health-oriented goals through the development of a therapeutic relationship.
Knowledge, Skills and Abilities:
- Masters of Social Work
- Two to five years experience in a community based human service organization
- Thorough knowledge of, and proficiency in, current assessment, psychosocial counseling techniques and group work skills.
- Thorough knowledge and understanding of community resources, preferably Guelph and Wellington County resources.
- Ability to set goals and get results within a set timeline
- Ability to multitask and work independently
- Effective written and oral communication skills
- Effective time management, decision making and organizational skills
- A good team member and supportive of interdisciplinary practice.
- High level of confidentiality
- Flexible towards change
Principal Job Duties:
- Maximizes the functioning and independence of clients by providing psychosocial counselling services to individuals, families and groups.
- Ensure clients receive community support, and health, social, and recreational services to which they are entitled, by acting as a client advocate.
- Acts as a resource person to Guelph CHC staff, clients, and community members by providing support and guidance particular to mental health.
- Ensures comprehensive treatment for clients by providing information to other Guelph CHC professionals when appropriate, on an ongoing basis, by participating in case conferencing, and by providing consultation on psychosocial issues to Guelph CHC staff.
- Ensures appropriate treatment is delivered and maintains client records, assessment notes, and correspondence within the safeguard of confidentiality.
- Contributes to the development of new programs that address community health needs. Collaborates with Guelph CHC staff and other agencies to identify, recommend and/or develop resources and materials, and participates in delivery of the program.
- Assists in developing funding proposals and budgets with respect to service delivery and protocols and policies as required by Guelph CHC.
- Supervises social work students in field placement practicums.
- Participates in community groups and committees relating to the development and delivery of health care and social services in the community.
