We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.

The Intensive Case Manager is to provide intensive case management services and outreach to an individual caseload of clients currently residing at our Special Care Unit or looking for support and admission to the Special Care Unit, which is a transitional bed-managed alcohol program for people living with an acute dependency on alcohol. The role of the ICM would be to support the clients admitted to the program in achieving documents, registering for housing lists, assisting in connecting the client to community-based supports and activities, assisting with goal development, life skills acquisitions, and supporting the Client Case Workers to assist the client to maximize supports offered within the Special Care Unit and assist them to secure housing and return to the community when they are ready.

This position is a unionized, full-time, temporary position. The contract end date is December 31, 2025. The rate of pay is $23.67 per hour. Hours per week are 40. The schedule is Wednesday Sunday, 12:00 PM to 8:00 PM.

Key Responsibilities:

  1. Community Outreach which could include meeting clients in shelters, hospitals or other community agencies to assess clients needs to determine suitability to referral to the Special Care Unit.
  2. Within the program, the ICM would be responsible for case management, goal development, skill development through a strength-based lens. Assess clients needs and goals in regards to alcohol management, life skills, socialization and housing needs and make appropriate referrals. This is done in line with the Social Work code of ethics and the Wesley case management standard practice of care.
  3. Assist clients to develop a tangible discharge plan that addresses their individual needs.
  4. Provide outreach support to clients upon discharge for a specified period of time. Some examples of support provided upon admission to the program would include the competition of VI-SPDATs, ensuring clients are on appropriate housing lists, assist in applying for appropriate funding whether it be OW/ODSP, pensions and Trillium Drug Program.
  5. As a client prepares for discharge the ICM would assist with referrals to community agencies to compliment the goals the client has been working on while with the SCU to ensure similar supports are available within the community.
  6. Assist in connecting clients with community activities for social and recreational purposes. To assist in avoiding social isolation and improve community connection.
  7. Assist with securing mental health and medical supports within the community prior to discharge.
  8. The ICM will assist with developing programs that will enhance life skills such as budgeting, meal planning and registering with food banks (upon discharge).

What you will bring:

  1. Post-secondary degree/diploma in Social Services, and registered with the college or relevant equivalent required
  2. Registered and in good standing with the Ontario Colleges of Social Workers and Social Service Workers.
  3. Certification in safe food handling, non-violent crisis intervention (NVCI), ASSIST and First Aid/CPR, P.I.E.C.E.S., and mental health first aid is required. Additional training and equivalents are considered an asset.
  4. Experience working with adults with addiction issues, mental health issues, and a history of homelessness. The successful candidate will have the ability to do this in a non-judgmental manner that supports the client in a client-centred manner. They will offer specialized and individual services to meet complex needs, andbehavioural and mental health challenges both within a residential setting and once discharged to the community.
  5. Experience in case management is a must, along with the use of and connection to community resources. The Intensive Case Manager will assist with program engagements, assessments, care planning, and appropriate referrals. This will assist the client to move through the program and connect to community resources.
