Requirements
As a Health Navigator at Sprout, you will serve as a crucial link between our patients and the healthcare services we provide. This role involves direct communication with patients, providing them with health and medical advice, guiding them to appropriate resources, and assisting in urgent health situations. You will also act as a liaison, helping to streamline appointment scheduling and follow-up care. This position demands a high level of empathy, medical knowledge, and the ability to manage multiple tasks efficiently in a remote setting.
You'll be responsible for:- Maintaining availability to respond to patient inquiries promptly within set hours; ensure callbacks or text responses are completed within 2 hours and no later than 24 hours.
- Providing immediate health and medical advice to patients via phone or text.
- Assisting in navigating and directing patients to appropriate healthcare resources both within Sprout and in the broader public health system.
- Facilitating urgent care directions in emergencies, emphasizing that this role supports but does not replace emergency services.
- Assisting with booking appointments when necessary, although primarily handled by other team members.
- Conducting research or referring patients to Sprout specialists when inquiries fall outside of personal scope.
- Communicating availability changes due to vacation or off hours proactively to patients and the Sprout team.
- Conducting Medical Intake Form review calls with Patients.
- A fully licensed Registered Nurse with the BCCNM.
- Excellent communication skills and empathy.
- Strong organizational skills and the ability to manage numerous requests efficiently.
- Proven ability to handle medical information discreetly and guide patients effectively.
- Comfortable working independently in a remote setting.
