Overview

Title: Project Manager

Status: Contract Full-Time

Timeline: Funding for this project runs from January 2025 to December 2026.

Shift: Monday-Friday from 8:30am-4:30pm. With the ability to flex hours depending on the needs of the organization.

Starting Rate of Pay: $80,000 - $90,000/year

Plus, Mileage Reimbursed at $0.48/km as applicable

We are seeking a highly organized and proactive Project Manager to prepare and align with emerging Ontario Health Teams (OHTs), we will seek to meet the needs of diverse and complex client populations through the development and testing of integrated roles for front line service delivery, education, case management and system navigation.

Primary Responsibilities

· Establish Project Leadership and Oversight

· Engage and Collaborate with sector and system partners

· Develop and implement required roles

· Develop Curriculum

· Streamline care management, system navigation and assessment.

· Establish solution-focused engagement with union partners

· Develop client profiles

· Test model to be in a state of readiness for expansion and growth

· Assess the training and professional development needs of community-based workers

· Implement a shared education expert and related training tools to support broader community-based staff education needs

· Build on the Attendant Care model in the development of a local training curriculum for staffing to expand and reduce competition for HHR in health care

· Combine intensive and preventative community services

· Build the foundation for collaborative, integrated service in a neighbourhood of care by addressing barriers to integration of services

· Create a template for a neighbourhood-based Service Access Hub

· Explore opportunities for future testing

· Test the impact of smart technology to help off-set HHR needs of persons served

Skills and Abilities

  • Exceptional verbal and written communication and listening skills.

  • Effective time-management skills and the ability to prioritize and organization workloads.

  • Ability to handle multiple tasks and similar deadlines.

  • Excellent organization skills to stay organized while travelling between and working with multiple programs.

  • Problem-solving skills used to obtain requested information or direct others on whom to contact or where to access the information.
