Job Description- Works under the supervision of the Program Director
- Collaborates with the interdisciplinary team to create personalized care plans based on the patient's unique needs and goals of care.
- Participates in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and occupational therapists
- Conducts physiotherapy assessment, consultation and planned interventions for patients living with a life-limiting illness
- Engages in skilled, compassionate communication with patients, families, and caregivers regarding mobility, function, and end-of-life care goals
- Educates and mentors patients, family and caregivers on strategies to maintain safe mobility and function in the home setting
- Maintains accurate and timely documentation of patient assessments, treatment plans, and outcomes, ensuring adherence to ethical and legal standards
- Master's degree in Physiotherapy
- Registered in good standing with the College of Physiotherapists of Ontario
- 3-5 years of experience working as an Physiotherapist preferred
- Valid Ontario driver's license and a reliable vehicle
- Ability to work autonomously and as part of an integrated health care team, with an emphasis on collaborative care planning
- Expertise in managing complex physical function and mobility issues in palliative care an asset
- Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.
- Familiarity with assistive devices, adaptive equipment, and environmental modifications
- Excellent communication and interpersonal skills
- Strong clinical skills and a passion for improving the quality of life for patients and their families
- Effective problem solving and critical thinking abilities
- Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting)
