Program Overview – Supportive Housing Programs: Relocation Program, Community Care
The Supportive Housing Department provides a variety of housing, community and support-based services to adults & seniors who have struggled with homelessness, mental health and/or addiction related challenges.
This Project/Program will aim to engage and support vulnerable tenants who are at risk of destabilization due to re-development and/or other unexpected events resulting in the loss of their housing. Tenant relocation services provide critical supports to enable people to avoid homelessness by stabilizing them as they are moved to new housing. They are designed to help individuals and households sustain their housing, connect within their new communities, and provide ongoing access to supplementary resources that ensure successful outcomes. It is ideal to provide tenants with individualized support plans that address and strategize around their unique situations and needs. Within a wraparound philosophy and framework, the team will aim to address housing options and stabilization, physical and mental health, employment, financial literacy and education, food access, home care, addictions, harm reduction etc,
What You Will Do- Carries out the management, operational responsibilities & oversight of programs, services, and initiatives associated with 2-3 buckets of relocation activities – Tenant Engagement/needs assessments/intakes and care plans 2. Stabilization Supports & Housing Help Services
- Provides ongoing orientation, training and supervision to all staff and conducts annual performance appraisals.
- Provides direction, coaching and performance management of frontline staff.
- Provides direction and support to staff regarding program issues, team building, and crisis incidents through individual meetings with staff and team meetings.
- Recruits and hires staff and assists in carrying out personnel functions within the programs in consultation with the Director.
- Works with the Director to ensure the planning and implementation of staff development.
- Implements ongoing promotion/awareness building on the services, supports, participants needs/wins
- Participates and represents the programs in existing and new external committees or networks in relevant sectors and internal staff committees as directed by the Director.
- Supports the Director in building and maintaining internal and external partnerships and collaborative activities.
- Reports risk, safety, security and liability concerns in a timely manner to Director
- Assists Director to implement solutions to improve overall safety, well being, transparency and effectiveness
- Develops, implements and oversees all program initiatives
- Develops and maintain policies and procedures with respect to tenant relocation and supports
- Organizes and facilitates community meetings and meetings between the City of Toronto, Funders and relevant staff at WoodGreen Community Care and Housing units.
- Develops and implements an effective evaluation strategy in collaboration with the Director.
- Develops and implements program/operational protocols in consultation with the Director.
- Ensures that organizational policies and procedures are carried out within all programs areas, services, and initiatives.
- Prepares and monitors program financial information and program budgets within the organization's administrative policies and procedures.
- Develops and implements data tracking systems – Pirouette/Treat/Alayacare/Ariett - manages budgets in collaboration with the Director; liaising with funders and completing funding reports.
What You Bring to the Team
- Master's degree or Bachelor's degree and equivalent experience.
- 5+ years' experience developing and implementing programs working with homeless and vulnerably housed individuals, older adults and seniors.
- Thorough understanding of homelessness, poverty, supportive & affordable housing, senior care, mental health and substance use, and a proven ability to develop effective services/supports for individuals and communities facing these challenges.
- Experience supervising staff, preferably in a unionized environment.
- Knowledge of RTA and occupancy agreements and property management.
