Salary: Dependent upon education and experience - salary range is $57,048 to $88,316 per year

FIREFLY is seeking a regular full-time Service Coordinator (Urgent Response Service) to join our team.

The Service Coordinator is responsible for leading inter-agency, multi-disciplinary teams of service providers in order to provide seamless service and build strong collaboration as the team works together to achieve the vision/goals identified by the youth/family in their service plan. Urgent Response Service is a time-limited rapid service response for children and youth with Autism Spectrum Disorder (ASD). The service is intended to address one specific high-risk behaviour with a focus on preventing further escalation of risk of harm to self, others, and/or property. Eligible families can receive up to 12-weeks of services and support which are specifically designed to address the behaviour of greatest concern. This position will require leadership skills in developing cohesive, collaborative community teams as they are asked to assist care teams in developing the urgent response service plan. Service Coordinators will demonstrate exceptional interpersonal skills and maturity.

Key Job Functions

  • Screen incoming referrals to determine eligibility for URS.
  • Development of an individualized URS Plan which will inform the single behaviour that will be supported through URS.
  • Facilitate meetings with relevant clinicians to refine and coordinate the delivery of the URS service elements and ongoing meetings of community teams supporting the client and family.
  • Support the development, monitoring and updating of the URS Plan.
  • Maintain program administration including accurate data collection and feedback from clients, families and service partners.

Experience with

  • Autism Spectrum Disorder (ASD) and relevant assessment intervention strategies as well as an understanding of mental health issues in children/youth with ASD to support and inform the development and delivery of a strong intervention plan.
  • Supporting families of children with multiple special needs
  • Working with team members from multiple sectors
  • A variety of provider agencies in the Kenora, Rainy River and Thunder Bay Districts
  • Goal writing; ideally, integrated SMART goals involving multiple-sectoral service providers

Able to work

Education/Experience/Skills/Knowledge Required

  • Undergraduate Degree in Human Services or equivalent education and experience
  • Minimum 3 years experience working directly with families, children and youth with multiple and/or complex special needs, with demonstrated experience in coordinating services including navigating multiple service systems/sectors
  • Recent demonstrated experience and competence working as part of a multi-disciplinary team, preferably working across agencies/systems in the provision of coordinated care
  • Demonstrated exceptional provision of high-quality, family-centred care, including an awareness of/willingness to learn, and demonstrate respect for the familys culturalbeliefs and traditions
  • Knowledge of relevant governing legislation and regulations
  • Assessment and service planning skills; specifically holistic approaches to service delivery
  • Knowledge of available local, regional and provincial resources
  • Advanced written and verbal communication skills
  • Demonstrated organizational and time management skills
  • Word processing and database management skills
