The Clinical Practice Coordinator (CPC) reports to the Director of Care and is expected to provide leadership and expertise to effectively support and advance evidence-based best practices within a long-term care environment.

Responsibilities:

  • Interprets nursing philosophy, and in collaboration with the Director of Care and the nursing team establishes and implements goals and objectives
  • Implements, evaluates, and supports new care programs and services based on best practices
  • Oversees Medication reconciliation process and fosters safe medication practices
  • Assists and takes part in the recruitment, retention, and performance management of nursing employees
  • Tracks and analyzes medication incidents on the MIRS portal
  • Supports team members during medication reconciliation process as needed
  • Assists with the development of policies and procedures consistent with resident evidenced-based care practices, CNO, RNAO MLTC, and other applicable legislation
  • Provides direction and hands-on support to Registered Staff in each program area (i.e. Skin Care and Wound Management, Falls Prevention, Continence Management, Infection Control & Prevention, End of Life)
  • Stays current with any new evidence best practices and research in Long Term Care and disseminates this information to management and team members
  • Takes the lead and facilitates utilization of best practice guidelines by working will all clinical team members to enhance resident's quality of care
  • Assists with the evaluation of all resident programs and services for relevance, quality, and effectiveness
  • Provides mentoring and coaching in regards to the clinical required programs (skin, falls, pain, etc)
  • Participates in and implements learning which develops one's own emotional self awareness and a person centered approach towards others
  • Provides mentoring and coaching in regards to compliance with facility policies and procedures, required legislation, and prevailing best practices)
  • Acts as a preceptor to nursing students and fosters their learning goals and needs
  • Acts as a key member of the on-boarding process of Registered Staff
  • Supports nursing students and new hires to ensure they feel welcome and safe in their practice
  • Completes special projects, audits, reviews, proficiency testing of clinical skills of nursing team members and all other duties as assigned

Qualifications:

The qualifications needed to join our family are as follows:

  • Registered Nurse with a current Certificate of Registration from the College of Nurses of Ontario, in good Current Certificate of Competence with the College of Nurses of Ontario as Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Comprehensive knowledge of evidence-based clinical practices and clinical skills
  • Comprehensive knowledge of long-term care concepts and philosophies
  • Knowledge of and understanding of program planning, evaluation, principles of adult learning, and organizational and clinical issues
  • Ability to identify training needs of the team and individuals
  • Proficiency in the Point Click Care Application
  • Knowledge and demonstrated ability to incorporate core competencies including communication, innovation
  • Teamwork and collaboration, and personal ownership
  • Ability to work within a team while recognising the skills and abilities of others as we are all individual
  • Excellent interpersonal and communication skills
  • Minimum of three (3) years of managerial experience in a long-term care setting as an asset
  • A gerontology certificate program or equivalent is an asset
  • Knowledge of RAI-MDS, AIS certified
  • Policy and Program Development
  • May be required to occasionally work days, evenings, nights, and weekends to support Registered Staff as necessary
  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory reference
