There is one Full-Time permanent Nurse Practitioner position available starting on December 02, 2024, working 37.5 hours per week.
This position will work for 2 days at the Village, 1 day at Safe Haven, and 2 days at Ben Road locations.
Your Role:
The Nurse Practitioner (NP) will provide comprehensive primary and preventive care services to seniors, youth, and children within the Agency. Responsibilities include conducting physical exams, diagnosing and treating illnesses, managing chronic conditions, and developing individualized care plans. The role involves offering preventive care services and collaborating with healthcare professionals and the interdisciplinary team in delivering high-quality clinical care to a diverse population. Demonstrates a passion and desire to work with a diverse multi-disciplinary team from an equity lens. Acts as a consultant to Agency leaders for special projects and initiatives.
Tasks:
Clients Care:
- Provides primary care services, including conducting physical exams, diagnosing and treating illnesses, and managing chronic conditions for clients of all ages.
- Develops and implements individualized care plans for clients, considering their unique needs and medical histories.
- Conducts mental health status assessments on clients as needs arise.
- Offers preventive care services such as immunizations, vaccinations, screenings, and health education to promote healthy lifestyles.
- Prescribes medications, including controlled substances.
- Performs minor medical procedures, such as suturing wounds.
- Educates clients about health conditions, treatment options, and preventive measures.
- Orders and interprets all laboratory tests, Orders diagnostic imaging tests.
- Advocates for clients and their families, providing support and resources to navigate healthcare systems and make informed decisions.
- Provides counseling on lifestyle changes, such as diet, exercise, and stress management.
- Manages chronic diseases such as diabetes, hypertension, and asthma, including monitoring client progress and adjusting treatments as necessary.
- Coordinates care with other healthcare providers to ensure comprehensive and continuous care.
- Consults regarding urgent health issues pertaining to clients.
Coordination:
1. Participates in community outreach programs to promote health awareness and provide services to underserved populations.
2. Collaborates with healthcare professionals, including physicians, specialists, and social workers, and pharmacists to ensure comprehensive and coordinated care.
Administration:
1. Maintains accurate and up-to-date patient records, ensuring compliance with healthcare regulations and standards.
2. Writes session notes/reports and contributes to care plans.
3. Engages in health policy development.
4. Participates in or conducts research to improve patient care practices, staying up to date with the latest medical advancements and integrating evidence-based practices into patient care.
Your Talents and Experience:
An undergraduate degree in Nursing is required, with a Master of Science in Nursing (MScN) preferred. Candidates must possess a current registration with the CNO as a Registered Nurse Practitioner. Five plus years of nursing experience in various primary care settings, including community health, and working with children, youth and seniors is preferred. Completion of a Prescribing Narcotics and Controlled Substances course approved by the CNO Council is an asset. Demonstrates a passion for antiracism and anti-oppression practices and creates an inclusive space for all members of the nursing team. Candidates should possess excellent written and oral communication skills suitable for diverse literacy levels, experience handling confidential and time-sensitive information, and excellent interpersonal skills. Proficiency in using computers and EMR software are required Valid First Aid & CPR Certification required.

When determining rate of pay for external hires, a candidates past related experience will be taken into consideration along with where others with similar experience currently fall within the band.
