The Position
We are seeking a Case Manager II to join our Guest Services team at Mercy House. In this role, you will work with individuals facing complex challenges and requiring a higher level of support. Referrals will come from fellow members of the Case Management team, including the Clinical Manager, and your focus will be on coordinating care and services for Guests with greater needs. Reporting to the Manager of Guest Services, your key responsibilities will include:
Key Responsibilities:
- Engage with individuals experiencing complex vulnerabilities and barriers to accessing care/services.
- Utilize a harm reduction approach to actively lower barriers for Guests seeking support.
- Coordinate service delivery with relevant providers and connect Guests to appropriate community services.
- Provide housing-focused support, prioritizing the social determinants of health.
- Document guest interactions in a shared electronic system to enhance collaboration across the organization.
- Offer person-centered, empowering support, working with Guests to build autonomy, foster identity, and engage in their own goal-setting and planning.
- Build and maintain professional relationships with Guests while adhering to clear, consistent boundaries.
- Collaborate with The Gathering Place's clinical and guest services team to provide a continuum of care, participating as part of a larger multidisciplinary team of co-located service providers.
- Engage in de-escalation, crisis management, and safety planning for Guests exhibiting behaviors that may hinder access to services.
- Work to establish an integrated service network for Guests with high acuity needs.
- Flexibility to work evenings, weekends, and holidays as needed.
- Conduct outreach visits when necessary.
- Perform other duties as required.
The Case Manager II plays a crucial role in delivering safe, effective, and efficient services to Mercy House Guests while adhering to organizational policies related to guest and workplace safety.
What You'll Bring
Qualifications and Skills:
- Bachelor's degree in Social Work
- Eligible for registration with NL Association of Social Workers
- At least two years of relevant experience in social work or a related field
- Strong knowledge of community resources and supports available for marginalized populations
- Excellent relationship-building skills, including conflict resolution, empathetic listening, and individual support
- Deep understanding and sensitivity to the connections between diversity and marginalization
- Strong interpersonal skills, with the ability to creatively engage individuals who may be socially isolated
- Proficient in professional documentation and record-keeping
- Demonstrated ability to navigate complex systems and advocate for clients
- Innovative and adaptable problem-solving abilities
- Valid driver's license and access to a personal vehicle
- Clear Record of Conduct and Vulnerable Sector Check
The Details
This is a full-time permanent position. The salary range is TGP 17, Step 1-6, $57,013.92 - $62,424.00. The Gathering Place – Mercy House offers health and dental, long term disability insurance, life insurance, matching RRSP plan, sick leave, and three week's paid vacation.
