How You Will Make an Impact
The Rapid Mobile Case Manager will provide shelter diversion supports to families who are at imminent risk of homelessness. The shelter diversion team assists families in identifying safe and appropriate alternatives to shelter accommodation.
- Short term case management and brief resolution services including financial supports, advocacy, information, referrals, mediation, and support with a housing search are offered
- Work closely with emergency shelters and partner agencies to leverage and coordinate the assets and services available in Hamilton to ensure that families have their individualized housing and support needs met
- Have a demonstrated knowledge of various governmental and non-governmental systems and an understanding of how to assist people in successful navigation through these systems
- An in depth understanding of trauma as it relates to homelessness and housing stability is also necessary
- Individuals will work from a strengths-based trauma informed model of service and an anti-racist/anti-oppressive framework
- To observe all Good Shepherd health and safety rules and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients, staff and others as well as to the property of the Good Shepherd Non-Profit Homes.
What You'll Bring to Our Team
- Minimum of 2 years' experience working in community outreach or emergency shelter
- Demonstrated knowledge of community resources as they relate to housing and homelessness
- Experience in an outreach model of service delivery and/or program development
- Excellent written and oral communication, team-building, negotiating and problem-solving skills required
- Current CPR and First Aid Certification
- Ability to work cooperatively and collaboratively within an interdisciplinary team
- Computer knowledge and skills
- Valid G Driver's license required
- Fluency in French and/or a language in addition to English preferred
