Transition Support Worker
Huronia Transition Homes is a multi-site, multi-program organization operating throughout Simcoe County, providing shelter, support, education and advocacy to women and their children who have experienced abuse. We work to end all violence against all women. We are committed to providing equitable services based in intersectional feminism and harm reduction philosophies.
La Maison Rosewood Shelter (a program of Huronia Transition Homes) is currently seeking a dynamic individual to join our team for a 7-month contract as a full-time Transitional Support Worker. The Transitional Support Worker provides advocacy, housing navigation and outreach support to women residing in shelter and in the community.
Key responsibilities:
- Provide individual, supportive case management, with a focus on securing housing, for women in La Maison Rosewood and Aquarius House;
- Provide housing search support and navigation for women in the community who have experienced abuse;
- Establish a safe, supportive, and non-judgmental environment to facilitate women's empowerment and ability to make change;
- Identify the individual needs and barriers for each woman and set objectives to address those barriers;
- Assist women in developing personal goals and action plans;
- Liaise with community agencies, landlords and housing providers to support women in service;
- Advocate on behalf of the women we serve, with their approval, in accessing relevant community resources;
- Assist women in making connections with community resources including housing, social assistance, medical services, basic needs resources and justice systems;
- Provide transportation and accompaniment to appointments as necessary;
- Represent agency on community-based HIFIS Working Group;
- Maintain relevant program data, as required, and report accordingly;
- Participate in program planning and evaluation as required, work collaboratively as a team member in the identification, development and recommendations of changes for improved service delivery
- Contribute to the development of an annual job plan in consultation with the Program Supervisor and/or Director of Operations
- Actively participate in staff and agency meetings
- Promote a healthy and respectful workplace
- Participate in applicable training, supervision and team meetings
- Remain committed to education and advocacy as a means to ending violence against women
- Perform other duties as assigned
- Abide by all policies, procedures, rules and regulations of HURONIA TRANSITION HOMES
Qualifications:
- Post-secondary education in a relevant discipline and/or equivalent life/work experience;
- An understanding of, and an ability to work with, diverse women including women from various socio-economic and cultural backgrounds.
- Knowledge of local community resources such as housing, social services, medical and justice systems, and familiarity with procedures for access;
- Ability to provide information, support, practical assistance, safety planning, resources, referrals and advocacy for women as needed;
- Strong problem-solving and conflict-resolution skills;
- Strong interpersonal, organizational and communication skills;
- Tact, sound judgment, and multi-tasking ability.
- Ability to work independently and participate as a team member;
- Communication skills in French or another language would be an asset.
- Flexibility and willingness to travel within Simcoe County, valid driver's license, and reliable vehicle.

Compensation is $26.70/hour.
