Program: Action Commitment Transformation (ACT)

Employment type: Permanent Full-Time (35 hours per week)

Reports To: Program Manager

Job Summary:

The ACT Case Manager (CM) works with highly vulnerable immigrant/refugee families/youth during the settlement process by providing a wraparound client-centred case management service. The CM is responsible for recruiting eligible families/youth and providing one-on-one case management, support and group services to a population who struggle through multiple barriers during their settlement process, with the goal of building and enhancing their capacity towards self-independence and self-reliance.

Key Duties and Responsibilities:

  • Provides recruitment through outreaching to at-risk/vulnerable immigrant and refugee families/youth
  • Conducts comprehensive intake, eligibility and needs assessment
  • Performs case management approach through setting action plan as well as the "wrap around" service plan with clients
  • Offers continuous case monitoring and follow up
  • Conducts home visits when necessary
  • Provides settlement counselling and settlement assistance and support to clients
  • Bridges clients to different services
  • Liaises and works closely with community agencies and partnering agencies
  • Conducts life skills, literacy and essential skills group sessions for clients
  • Provides group services, case conferencing, supported access and transition support to external services
  • Ensures all services meet the accreditation standards
  • Maintains up-to-date case record
  • Responsible for report writing and other administration work
  • Maintains statistical records for accountability, funding application/report and program development purposes
  • Attends case conferences, supervision and team meetings regularly
  • Attends division staff meeting and training opportunities as required
  • Works closely with staff members of other S.U.C.C.E.S.S. divisions/offices in providing joint support to individuals and families
  • Provides support to agency wide fundraising initiatives
  • Performs other tasks as assigned by Senior Program Manager or Program Director

Education, Training and Experience:

  • Bachelor degree or Diploma in Social Work, Psychology, or related discipline
  • Two (2) years of family/youth work experience and/or settlement work experience with vulnerable population is preferable
  • Experience in group facilitation
  • Demonstrates knowledge about local community resources for at-risk/vulnerable immigrant population
  • Demonstrates experiences on conflict resolution, case management and basic counselling skills
  • Demonstrates strong understanding on case planning and case management principles and techniques
  • Demonstrates good knowledge of issues vulnerable immigrant families and youth are facing, including social, cultural, emotional, mental health, health and sexual orientation issues
  • Demonstrates language and cultural proficiency that is applicable to the various source countries of vulnerable populations in their service area
  • Demonstrates knowledge and experience in cross-cultural communication and service methods appropriate to clients with low literacy and low English skills

Job Skills and Abilities:

  • Excellent verbal and written communication skills
  • Second language in Spanish is considered an asset.
  • A good team player with internal and external stakeholders
  • Ability to deal effectively with people from diverse backgrounds
  • Excellent program planning, time management and organizational skills
  • Proficiency in the use of Microsoft Office Suite and basic internet skills
  • Comfortable in performing home visit/outreach
  • Multi-tasking and flexible

Additional requirements:

  • Criminal Record Check – Vulnerable Sector Check (submitted upon hiring)
  • Flexible to work on evening, weekends and holidays
  • Available to travel occasionally
  • Available to attend to crisis intervention which may extend beyond the normal office hour occasionally
