Registered Dietitian – Humber River Family Health Team (Toronto)

The Humber River Family Health Team (HRFHT) is a non-profit organization.
We provide primary health care, including chronic disease management and health
promotion and disease prevention, to a rostered population of patients in the northwest
Toronto area.

About the position:
We are seeking an energetic and experienced Registered Dietitian to provide primary
care at our 2050 Weston Road location. The Dietitian provides primary health care functions and works in collaboration with an interdisciplinary team.
Duties and Responsibilities
- Screens, prioritizes and processes client referrals.
- Reviews client clinical information obtained through referrals, laboratory reports and health records.
- In collaboration with Physicians, Nurse Practitioners and other Allied Health professionals, implement and discuss appropriate care plan with the client & family/care giver based on best practices.
- Provide teaching to clients, explaining the relationship between their intakes and medical status. Alter teaching based on client's nutritional needs, revising treatment protocols and thereby facilitating the client's ability to alter intake to manage their medical condition.
- Collaborate with other health care providers to provide the best care for the client.
Education/ Experience/Knowledge:
- Completion of a four-year undergraduate university degree from an accredited university program in food and nutrition or equivalent.
- Completion of accredited internship OR Combined Masters Practicum Program.
- Certified Diabetes Educator (CDE) or working towards CDE preferred.
- Registration with the College of Dietitians of Ontario.
- Working knowledge/skill with adult education techniques.
- Working knowledge of Electronic Medical Record (EMR) and required computer skills.
- Able to work flexible hours (weekend and evening work required).
Job type: Full-time, Permanent
Remuneration: $74,148.00 annually.
