YOUTH TRANSITION WORKER

GENERAL DESCRIPTION
The Youth Transition Worker is a full-time regular program position. The Youth Transition
Worker provides direct services to youth who are approaching the age to be out of care of the
Agency. The Youth Transition Worker co-ordinates a transition phase for youth and
provides services to ensure the needs of the youth are met according to First Nation standards
and Kitapinoonjiiminaanik Family Services philosophy.


RESPONSIBILTY
The Youth Transition Worker reports to the Service Navigator in all aspects of job functions.
The Youth Transition Worker will exercise the following duties under the supervision of the
Service Navigator.

QUALIFICATIONS


  1. Bachelor of Social Work Degree. However, a combination of skills, education
    and experience may be considered;
    2. Thorough knowledge of the Child and Family Services Act and Regulations,
    particularly in the applications relating to Customary Care of the
    First Nations, and provincial court procedures in relation to Part III;
    3. Expert knowledge of Customary Care and the philosophy of service development
    and delivery; the communities and family structure, as well as local First
    Nation customs and traditions;
    4. Working knowledge of the administrative structure and operations of
    Kitapinoonjiiminaanik Family Services including the service delivery model,
    policies, procedures and guidelines;
    5. Strong commitment to help Anishinaabe Youth and their families by providing
    services in ways that respect Anishinaabe cultural and spiritual practices;
    6. Commitment to provide services in the Anishinaabe context of extended family
    and community involvement;
    7. Knowledge of and experience in traditional and contemporary healing practices
    with experience working with Elders, Healers, and Clinicians;
    8. Expert knowledge of the Kitapinoonjiiminaanik Family Services case
    management model, its procedures and documentation requirements;
    9. Proven ability to meet deadlines;
    10. Ability to take direction and work within the Agency's philosophy, policies,
    procedures, and guidelines, as well as provincial regulations and standards;
    11. Ability to work within a multi-disciplinary Child Welfare team;
    12. Working knowledge of group dynamics, consultation, and conflict resolution
    techniques;
    13. Ability to establish and maintain trusting relationships with Youth and their
    families;
    14. Ability to establish and maintain purposeful relationships with Management,
    staff, Alternative Care Home providers, Family Services Committees, and all
    other relevant service providers;
    15. Sound knowledge of human behaviour theories with the ability to apply in
    assessment and planning;
    16. Excellent interpersonal skills with a proven ability to work effectively with Youth
    of a wide range of developmental, social, and family backgrounds;
    17. Superior oral and written and communication skills;
    18. Excellent time management, planning, organization, problem-solving, decision-
    making, and liaison skills;
    19. Experience working with a computerized database;
    20. Committed to always ensure the highest level of confidentiality;
    21. Ability to flex daily work hours as determined by the Resource Manager;
    22. Must possess a valid Ontario Driver's Licence and be willing to travel;
    23. Must provide a clear Criminal Records Check and Driver's Abstract; and
    24. Ability to speak Ojibway is preferred and a definite asset.
