WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $29.57 per hour (Level 1 of the Direct Service - Regulated Grid);
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $36.71 per hour
    • Internal applicants will be placed on the grid based on their seniority
  • When you'll work – 70 hours over a two-week period; Option of a four day work week (Monday-Thursday, 8:00am -5:15pm) including a 30 min unpaid lunch
  • What your benefits will be - After a 90-day waiting period, will be enrolled in the healthcare benefits plan
  • Your pension plan – Will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)


HOW YOU WILL BE MAKING A DIFFERENCE:

  • Conducts physical and psychiatric assessments and makes appropriate referrals to community physicians and resources
  • Assesses and documents medication effectiveness, complications, and side effects
  • Identifies, develops, and maintains strong links with a wide range of community partners/resources
  • Organizes, coordinates, and administers depot injections
  • Reviews and audits completion of Medication Administration Records (MARs) completed by staff within the home, completing incident reports and taking appropriate measures in response to medication errors to ensure health and safety of affected tenants
  • Assists clients in making informed choices about health measures such as medications, immunization, palliative care, disease management, etc.
  • Supports the education and Medication Management policy and procedure training of CHO private home staff
  • Assists clients in taking responsibility for and managing their health needs according to available resources and personal skills
  • Collaborates with community agencies to maintain coordination in the treatment process
  • Identifies high risk clients and assess the need for crisis intervention, prioritizing the safety of clients
  • Utilizes harm reduction principles to identify, reduce, and remove risk factors
  • Works collaboratively and cooperatively with clients, families, peers and other care providers in order to provide safe, quality care, that maximizes benefits to the participant
  • Interacts and provides support in a manner that demonstrates sensitivity to client diversity (e.g. culture, race, age, sexual orientation, gender, beliefs and values)
  • Assists in motivating clients to learn new behaviours and develop coping skills
  • This position may also provide nursing support to additional programs within our supportive housing programs


WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.


We require the following qualifications:

  • Diploma in Nursing as a Registered Practical Nurse (RPN) (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Current Registration from the College of Nurses of Ontario
  • Valid Driver's Licence, adequate insurance and access to a vehicle to carry out job duties
  • Demonstrated ability to attend work regularly


We prefer the following qualifications:

  • Excellent communication and interpersonal skills
  • First Aid & CPR certification
