Job Description: Registered Nurse (RN) Maternity Leave

Schedule: Monday - Friday 8:00-4:30 pm

The MacEwan University Health Centre is a new state-of-the-art primary care clinic, located in downtown Edmonton, focused on the Patient-Centered Medical Home model which provides patient-centered, comprehensive, coordinated, and accessible care based on the patient's needs. Reporting to the Clinic Manager, the RN will support physicians and is a team leader of the nursing team. Together with the team, the RN is responsible for providing care and support to a designated panel of patients. Most importantly, the dedicated pod will provide care to the under-served LGBTQ community. It will be important to understand the unique health needs and concerns and the diverseness of the LGBTQ community and provide a welcoming and inclusive health care environment. Additionally, the RN will also have a large teaching and mentoring component to the role whereby they will participate in the education of physicians, residents, LPN's, CHA's and non-physician learners. There will also be collaboration with other health care professionals/providers to help achieve our model of Patient Centered Care.

Duties and Responsibilities

  • Triage patients during Covid-19 pandemic.
  • Create and update clinical policies.
  • Coordinates daily huddles.
  • Contributes to shared goal setting, education, and management of chronic disease conditions for assigned patient panels.
  • Performs daily procedures for scope of practice such as management of minor acute and stable chronic conditions on protocols.
  • Reviews physician task list and manages test results/outstanding issues by protocol.
  • Assists with form completion for physicians.
  • Manages labs and screenings by protocol.
  • Manages prescription renewals by protocol.
  • Coordinates allied health resources as required.
  • Telephone triage and advice to patients and families.
  • Reviews clinic data regularly and uses the data to aid in the proactive management of patient populations.
  • Performs in depth clinical care, wound care, and procedures, including injections.
  • Prenatal Checks and Obstetrical visits.
  • Well baby checks.
  • Prenatal appointments
  • Cognitive screening
  • HIV Pre-Exposure Prophylaxis (PrEP)
  • STI screening
  • Positive STI review/treatment and contact tracing
  • COWS (Suboxone start-opiate withdrawal assessment)
  • Blood pressure/Diabetes/CDM Assessment
  • Immunization
  • Emergency Response (Code Blue)
  • Procedure assistance
  • Task Management
  • Walk in Assessment Triage.
  • Pharmacy liaison.
  • Care coordination.
  • Responsible for patient education.
  • Calls new patients prior to their visit for a detailed health history.
  • Performs home and group visits as required.
  • Responsible for the orientation, training, and mentoring of CHA's and LPN's.
  • Teaching Residents – orientation, guidance, procedure assist, allowing them to observe.
  • Works with the clinic manager to develop and operate quality control and CQI programs.
  • Supports Significant Event identification and analysis processes.

Qualifications

  • Completion of accredited nursing education program required.
  • Active or eligible for registration and practice permit with a relevant regulatory college.
  • Criminal Record and Vulnerable Sector check
  • Current Basic Cardiac Life Support – Healthcare Provider (BCLS-HCP)
  • Up to date vaccinations.
  • Minimum 2 years' experience.
  • Primary Care/Family Practice experience preferred.
  • Clinical Knowledge of and experience working with the LGBTQ community is preferred.
  • Experience with leading and collaborating in teams.
  • Knowledge of Primary Care Medical Home Model.
  • Experience with leading a team, teaching and mentoring.
  • EMR Experience.
  • Proficient in MS Office Suites.
  • Excellent communication and time management skills.
