**Key Responsibilities**
*Client Care Management & Experience*
- Develop detailed, personalised care assessments for every client and keep them updated
- Ensure every new caregiver receives a thorough introduction to their client, fostering positive relationships
- Conduct supervisory visits to ensure caregiver fit and the highest quality of care
- Collaborate with the client's family and healthcare team for seamless continuity of care
- Educate clients on self-care and evaluate their responses to care, making adjustments as needed
- Monitor and adapt care plans based on risk factors, client goals, and outcomes
*Caregiver Leadership*
- Lead and inspire your caregiving team by setting clear goals, coaching in real-time, and offering ongoing education
- Ensure caregivers provide high-quality care that leads to client satisfaction
- Oversee staff performance, ensuring adherence to policies and best practices in all client interactions
- Manage onboarding and training programs to promote a continuous learning environment
- Conduct supervisory visits to evaluate caregiver performance and ensure job satisfaction
*Scheduling Collaboration*
- Collaborate with the scheduling team to ensure timely, accurate handoffs of client files
- Communicate changes in client needs and caregiver assignments to maintain quality care
- Assist with scheduling as needed, ensuring that client coverage is always secured, especially in urgent situations
- Monitor and maintain compliance with clock-in/clock-out procedures, ensuring caregiver accountability
*Consult Conversion*
- Conduct client consultations, assessing their needs holistically and achieving an 80% conversion rate
- Build strong relationships with community leaders and organisations, creating a network for referrals
- Provide expert advice during consultations, ensuring clients and their families have all the information they need
*Administration*
- Maintain accurate, up-to-date documentation of care plans, employee files, client records, pay, and billing information
- Review and update client records regularly to ensure completeness and accuracy
- Conduct quarterly audits to maintain the highest standards of documentation
- Ensure compliance with all relevant national and provincial health, employment, and safety regulations
-Collaborating with the interdisciplinary team - OT, PT, SLP, Ontario health at home care coordinators
**Qualifications**
- -Nursing Diploma & CNO Registration Required
- - Nursing background (RN or RPN) required
- - Reliable vehicle and valid driver's licence
- - 1-2 years of nursing experience preferred
- - Strong computer skills, including proficiency with Office 365
- - Excellent written and verbal communication skills
- - Highly organised, with the ability to manage multiple tasks and set clear goals
- - Customer service and leadership experience are considered an asset
