Job Description

As a member of our inter-professional team, the Nurse Practitioner will utilize clinical expertise and in-depth knowledge of children, adolescents and their families to plan and evaluate patient care. The successful candidate will provide leadership and promote excellence in nursing by providing expertise in clinical practice, consultation, education and research in the area of children & adolescent and the transitional aged population. The Nurse Practitioner will integrate research evidence into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.

The principal responsibilities of this role are to identify, assess, consult and provide clinical intervention to children & adolescent and transitional aged population with Mental Health. This will include providing specialized assessment, coordination and treatment for individuals living with Mental Health. The Clinician provides assessment supportive counselling, education, and resources to children and adolescent and families within the Urgent Care Clinic and helps clients understand and address Mental Health related problems. The clinician is expected to work collaboratively with an inter-professional team within the Mental Health Program and to establish and maintain partnerships with related community organizations. This role will be part of the Child & Adolescent program within Mental Health.

Qualifications

Halton Healthcare Services is seeking individuals who are leaders in their chosen profession. The successful candidate must be a natural team player with effective communication skills. They must be patient focused, caring, respectful and fiscally responsible. The successful candidate will have:

  • Master's Degree Program in Nursing/Master of Science in Nursing or recognized equivalent is preferred
  • Current registration in the Extended Class with the College of Nurses of Ontario is required, preferably with the Primary Health Care specialty certificate
  • Nurse Practitioner's Certificate from a recognized program
  • Current membership in professional association such as CNA, RNAO or NPAO preferred
  • Five years of experience working in the field of Mental Health, with a focus on Children & Adolescent and Eating Disorders.
  • Expertise in conducting bio-psychosocial assessments and strong formulation skills to design person-centred, strength-based change strategies that reflect evidence-based practices and clinical experience.
  • Experience in motivational interviewing and trauma-informed care.
  • Provide integrated model of care that will include linking clients to primary care and/or fostering existing relationships with primary care to ensure client needs are being met.
  • Knowledge of individual, family, group, and crisis counselling methods.
  • Demonstrated skill in working with families of those who have mental health & substance use disorders.
  • Demonstrated skill and knowledge about concurrent mental health and substance use disorders.
  • Substantiated experience working with inter-professional teams, under pressure and within a fast-paced hospital setting.
  • Demonstrated understanding of diverse cultures and the principles of cultural safety.
  • Experience in matching treatment services and interventions to clients' and their families' unique circumstances whenever possible.
  • Work in partnerships with primary care and community providers.
  • Ability to work independently.
  • Proficient with MS Office applications and related electronic health care platforms
  • Satisfactory employment and attendance record required
  • Experience in the provision of psychoeducation and psychotherapy is preferred

Additional Information

Hours: Various rotating 7.5 hour shifts; 0830 - 1630, 1100 - 1900 (Hours are subject to change based on operational needs)

Salary: $58.800 to $72.010 per hour
