North Simcoe Muskoka Specialized Geriatric Services (NSM SGS) is a regional program under the leadership of Waypoint Centre for Mental Health Care. NSM SGS was established in 2016 to advance the care of frail older adults and their caregivers in North Simcoe Muskoka. With a mandate for leadership, clinical service, education and mentorship, research and ethics as well as advocacy, the program collaborates with a broad variety of partners from within and outside the NSM health system to improve patient outcomes, build capacity and foster system change.

In this advanced practice nurse role, this Clinical Nurse Specialist (CNS) in Geriatric Mental Health will bring expertise in geriatric mental health (including responsive behaviours) and specialized geriatric services to regional partners to improve patient outcomes, build capacity and foster system change. More specifically, this CNS will provide support to the Waypoint Centre for Mental Health Care Horizon Program and the County of Simcoe Behavioural Support Transition Unit (BSTU) to advance clinical care and outcomes, build team capacity, develop programs and standards and create partnerships with key regional organizations. The roles of the CNS will include clinical practice, education/mentoring and leadership.

Please note: This position will be located across sites in the North Simcoe sub-region. Travel will be required for this role.

KEY ACCOUNTABILITIES:

Implements the recognized competencies of an advanced practice nurse role: clinical practice; leadership and management; education/mentoring; and research.
Provides excellence in clinical practice in the care of older adults and their caregivers, including leading the development and implementation of clinical best practices, practice guidelines, standardized protocols, etc.
Supports the development, implementation and evaluation of clinical services and programs to improve the quality of care provided to older adults and their caregivers.
Provides consultative support to local SGS teams and partners to advance clinical practice and build system capacity; mentors and coaches interprofessional teams in complex cases.
Educates and mentors members of SGS interprofessional teams and other clinicians across the region to build regional and local capacity in the care of older adults and caregivers.
Promotes a learning environment for older adults, caregivers, health service providers, health care students, leaders and policy makers.
Uses research and leading practices to inform clinical care, education and planning. As appropriate, leads or participates in quality improvement initiatives to influence clinical practice, planning and/or policy.
Participates in data and information analysis to identify and address gaps in care, issues in practice, risk issues and/or concerns impacting the safety of older adults or their caregivers.
As appropriate, represents the NSM SGS Program locally, regionally and provincially.
Builds strong collaborative relationships with key internal and external partners and stakeholders.
Manager has the right to assign additional responsibilities

REQUIREMENTS/QUALIFICATIONS:

Master's Degree in Nursing or other health related science Required
Current registration as a Registered Nurse in good standing with the College of Nurses of Ontario Required
Current certification in Gerontology and/or Geriatrics e.g. academic institution, GNC(C) from the Canadian Nurses Association Required
Valid Ontario driver's license and operational vehicle
P.I.E.C.E.S., Gentle Persuasive Approach (GPA), Positive Approaches to Care (PAC), Montessori training
Membership in professional associations and/or interest groups preferred
Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
Demonstrated leadership, conflict resolution, interpersonal and organizational skills
Proven written and verbal communication skills
Proven ability to adapt readily to change and reflects an optimistic and positive attitude
Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation)
Preference for proficiency in French/English language skills.
