SALARY: $48.21 - $64.50
POSITION DESCRIPTION:
As a member of an inter-professional healthcare team, you will be providing direct patient care to identify, solve and prevent drug therapy problems. Other responsibilities include participating in our oncology service, patient education, providing drug information and/or education to nursing and medical staff, ensuring patient medication safety and supporting the drug distribution services.
HOURS:
The successful candidate must be available to work all tours applicable to the department. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department.
SKILLS & QUALIFICATIONS REQUIRED:
Graduate from an accredited post-secondary Pharmacy Program (i.e. B.Sc Phm, M.ScPhm, PharmD)
Current registration (Part A) in good standing with the Ontario College of Pharmacists
Previous hospital pharmacy experience and/or hospital residency, especially within an oncology setting, is a definite asset
Knowledge of pharmaceutical care process, disease management, and clinical pharmacology
Familiarity with evidence-based practice; including strong clinical assessment in identifying and solving drug related problems;
Demonstrated team work in a patient care setting
Demonstrated organizational, time management, critical thinking and deductive reasoning skills
Demonstrated communication skills in providing pharmaceutical service to patients; including clear and concise verbal and written communications
Demonstrated skills in computerized pharmacy systems (ie. Cerner, Pharmnet preferred) and basic Microsoft Office programs (Word, Excel, Outlook)
Demonstrated knowledge of professional standards
Adaptability and flexibility in a dynamic, busy and sometimes stressful environment.
Ability to attend work on a regular basis in accordance with STEGH's Attendance at Work Program
