SUMMARY OF DUTIES
The Nurse Practitioner (NP) for Macassa Lodge is responsible for providing or facilitating resident centered, primary health care in collaboration with the resident, family and interdisciplinary team. The Nurse Practitioner will be an educator, consultant, leader and advocate to the residents, families and interdisciplinary team.
GENERAL DUTIES
Using advanced clinical skills the NP will provide direct clinical care, education, consultation and treatment in the area of geriatrics to support our residents in the Lodge who may experience acute episodic and/or chronic health conditions.
Partner with the Medical Director, Attending Physicians, Interdisciplinary Care Team and the Leadership team at the Lodge to enhance the circle of care for our complex elderly residents and their families.
Work autonomously and collaboratively to provide individual, person centred and evidenced based health care to residents within our Lodge.
Perform complete, in depth and comprehensive assessments including functional and mental status assessments in accordance with expectations outlined in the Long Term Care Homes Act 2007 or as necessitated by resident condition, family request or team referral.
Report changes in chronic illness to the physician or Medical Director
Episodic care for common primary care conditions, in conjunction with Medical Director, attending physician and the interdisciplinary team.
Assess, advise and treat regarding common conditions experienced by the elderly.
Monitor chronic medical conditions, in conjunction with the Medical Director, to maximize resident function and to maintain compliance with Ministry of Health and Long Term Care standards for LTC.
Assess for, recommend and/or implement strategies to promote health and optimal level of functioning for residents at the Lodge.
Assess the need for and provide education to resident, family and staff.
Promote healthy lifestyle choices.
Participate in the Lodges' ongoing Quality Improvement initiatives and programs.
Perform other duties as assigned which are directly related to the responsibilities of the position.
Work in accordance with the provisions of applicable Health and Safety legislation and all City of Hamilton corporate and departmental policies and procedures related to Occupational Health and Safety.
QUALIFICATIONS
1. Baccalaureate in Nursing.
2. Current Registration with the College of Nurses as Registered Nurse in the Extended Class.
3. Additional education, experience and/or certification in Gerontological Nursing are an asset.
4. Knowledgeable about primary health care issues, chronic disease management, behavioural health in older adults and palliative care in long term care facilities.
5. Demonstrated excellence in interpersonal and communication skills.
6. Demonstrated ability to work as a member of an interdisciplinary team.
7. Knowledge and familiarity with use of computers is necessary.
8. Demonstrated ability to prioritize and work effectively in a fast paced environment.
9. Demonstrated ability to welcome change and manage it both effectively and innovatively.
10. Proven leadership skills, strong organizational, analytical, conceptual and communication skills.
11. Demonstrated commitment to ongoing learning.
THIS POSITION REQUIRES A VALID CLASS "G" DRIVER'S LICENCE AND PROOF THEREOF IS REQUIRED AFTER HIRE.
