About the Opportunity

We currently have an opportunity for two (2) Registered Nurses to join our Outpatient Program in Toronto, ON, on a full-time, permanent basis.

Hours of work: Primarily Monday to Friday (9-5PM), with two (2) scheduled 12-hour, Inpatient weekend day shifts once every four (4) weeks to commence at a future date. A total of 225 hours in a six-week period.

As a Registered Nurse - Outpatient Program, you will collaborate with other members of the program to provide trauma-informed clinical care to Casey House clients. Reporting to the Clinical Lead, Inpatient on an interim basis, you will provide transitional primary care services, and respond to acute needs of our clients who already have a primary care provider. As the Outpatient Program evolves to meet the needs of our community, you are expected to play a key role in the development of expanded harm reduction services, including work in supervised consumption services, safer supply programs and HIV prevention initiatives.

As an integral member of a multidisciplinary team, you will work with clients and their support networks, from a holistic and client-centred perspective, to deliver focused group and individual interventions to improve emerging and/or chronic health issues.

Your responsibilities will entail, but are not restricted to:

Performing primary and urgent episodic nursing duties as they relate to: physical and psychosocial assessments, phlebotomy, harm reduction care for substance use and addiction, HIV/STI testing and treatment, wound/foot care, client screening, etc.
Providing triage and intake services for existing and potential clients
Liaising with Inpatient team to provide barrier-free transitions between the Inpatient and Outpatient Programs
Acting as a resource for the interdisciplinary team by providing information based on nursing theory and practice related to patients/clients with complex health care needs
Participating as group facilitator/co-lead for Outpatient program specific groups for client education or interventions
Inpatient care activities including: palliative care, management of central venous lines, post-surgical/radiation/chemotherapy care, rehabilitative activities, harm reduction and mental health interventions, and client education.
Provide direct nursing care and support to clients at the Blue Door Clinic as part of the cross-agency collaboration to serve PHAs without health insurance.
Collaborate with the Nurse Practitioner and other healthcare providers at the Blue Door Clinic to ensure comprehensive and coordinated care.
Mentoring students, volunteers, and new staff.
Advocating for patients/clients, to protect and promote their rights to dignity, respects, privacy, confidentiality, autonomy and access to information.

Please note, given the weekend day shift requirement in the future, further orientation for this role will include up to a one month term working on the inpatient unit on a 8-hour day/night shift.

About You

In order to qualify for this role, you will need a Nursing Degree or Diploma, as well as a minimum 2 years of Registered Nurse experience.

The following qualifications will ensure your success in this role:

Current member in good standing with the College of Nurses of Ontario (CNO) without restrictions
Valid CPR certificate
Good understanding of the principles of client-centred care (autonomy, self-determination), with the ability to work from both a harm reduction and anti-oppressive framework
Expertise in physical, mental health, and addictions assessments & adapting care planning to client environments of care
Membership in RNAO, CANAC and/or ANAC (asset)
Experience in HIV/AIDS, mental health and addiction care (asset)
Crisis intervention training or willingness to pursue
Proficiency in the use of computer technology (health systems, Microsoft, etc.)
Exceptional verbal and written communication skills
