Job Description

TRC is removing barriers and increasing access to health and wellness services for young people ages 12-24 with a new initiative, Integrated Youth Hub. By offering integrated mental health care, substance use services, physical and sexual health care, youth and peer support and social services, TRC is making it easier for young people to find support in their communities.

The Social Worker provides clinical support to Hub staff and promote interdisciplinary services and collaboration to support the identified outcomes. They act as a clinical resource to staff, students and inter-professional team members, facilitate effective problem solving and promote collaboration. They interpret, implement and monitor policies, procedures and standards related to client care in person and in a possible virtual environment. They collaborate with TRC's interdisciplinary team members, the backbone support team, other Integrated Youth Hubs, and health and community providers to ensure timely and seamless delivery of services. They identify, coordinate and promote evaluation, research and quality improvement initiatives in collaboration with the provincial backbone and research leads for IYS in NB. The Social Worker participates in team and/or local committees and internal and external case conferences. They also participate in the Integrated Youth Hub provincial network and related working groups and events.

Key Duties and Responsibilities

  • Provides clinical support, mentoring, coaching and consultation to the Hub's interdisciplinary service providers, students, interns and partners in the clinical care of young people.
  • Collaborates in problem solving clinical cases, providing and/or recommending interventions, programs, resources and literature to promote continuity of care, effective treatment and resource utilization.
  • Oversees daily workflow for walk-in counselling and monitors caseload allocation.
  • Provides clinical support and supervision for walk-in counselling, with an emphasis on evidence-informed and culturally responsive practice.
  • Emphasizes the importance of providing integrated services for clients.
  • Acts as a resource for mental health and substance use issues
  • Develops partnerships with other facilities, agencies and community resources to best meet client needs.

Qualifications

  • University degree in Social Work, and a registered Social Worker in good standing with their licensing association.
  • 3 years of related experience in their clinical specialty area; community addiction and/or mental health.
  • Experience working in a multi-disciplinary team setting.
  • Demonstrated leadership abilities and supervision experience.
  • Experience working directly with youth, and multiple partners in the care of youth.
  • Written and spoken competence in English. French is considered an asset.
  • Experience with developing policies and procedures and designing and delivering community programs.

  • Flexible work hours including some evenings and weekends.
